摘要
目的分析医生、护士、药师和患者用药错误(medication errors,ME)情况,讨论减少ME的措施。方法收集某院2009～2011年ME案例,分析其错误类型和发生原因。结果 ME由医生引起的有67例(42.95%),护士引起的有46例(29.49%),药师引起的有25例(16.02%),患者引起的有18例(11.54%);131例ME为B、C、D级别,超说明书用药中大多为E、F级。结论建立用药安全报告系统、智能化药物治疗系统、实施PDCA循环管理方法、使用条形码扫描技术、加强患者用药教育,可有效减少ME。
Objective To analyze the medication errors （ME）of doctors, nurses, pharmacists and patients and discuss the measures for reducing the errors. Methods The medication error cases in a hospital during the period from 2009 to 2011 were collect- ed, and error types and causes were analyzed. Results Sixty-seven ME cases （42. 95% ）were caused by doctors ,46 cases （29.49%）by nurses,25 cases （ 16.02% ） by pharmacists and 18 cases （ 11.54% ） by patients. The most of medication errors were B, C and D class. The off-label drug Uses were mostly in E and F class. Conclusions The medication errors can be effectively reduced by establishing a safe medication reporting system, an intelligent prescription screening system, an implementation of PDCA circulation management, a bar-code scanning technology and an enhanced patient medication education.
