腰-硬联合麻醉对老年患者下肢手术中血流动力学的影响

Effect of combined spinal with epidural anesthesia on hemodynamics in elderly patients with lower limb surgery

目的观察腰-硬联合麻醉对老年患者下肢手术中血流动力学的影响。方法 50例下肢手术的老年患者按随机数字表法分成腰-硬联合麻醉组(A组)和腰麻组(B组),比较两组患者麻醉前、中、后收缩压(SBP)、舒张压(DBP)、心率(HR)、血氧饱和度(SpO2)的变化情况。结果 A组麻醉中、手术结束时各项观察指标均稳定;B组麻醉中SBP、DBP下降,HR升高,与麻醉前及A组比较,差异均有统计学意义(P<0.05)。结论腰-硬联合麻醉在老年患者下肢手术应用中循环稳定,安全可行。

Objective To observe the effect of combined spinal-epidural anesthesia （CSEA） on hemodynamics in elderly patients with lower limb surgery. Methods Fifty cases were randomly divided into group A （CSEA） and group B （Spinal anesthesia, SA）. The changes of SBP, DBP, HR and SpO2 were compared between the two groups before, during and after the anesthesia. Results The hemodynamics of the group A was stable during the operation while SBP and DBP was decreased （P 〈 0.05 ） and HR was elevated （P 〈 0.05） in the group B. The differences were statistically significant between the two groups. Conclusion CSEA is safe and feasible in elderly patients with lower limb surgery.
