细胞外信号调节激酶-2在结肠癌中的表达及临床意义

摘要 目的研究细胞外信号调节激酶-2(ERK-2)在结肠癌组织中的表达及临床意义。方法选取山东省临朐县人民医院2008年1月~2010年12月经手术病理证实的结肠癌患者79例,采用免疫组化链霉菌抗生物素蛋白-过氧化酶(streptavidin-peroxidase,SP)法检测正常结肠组织、癌旁组织和结肠癌组织中ERK-2蛋白阳性表达情况;分析ERK-2蛋白表达与结肠癌临床病理特征及预后的关系。结果①结肠癌组织中ERK-2蛋白阳性表达率[75.9%(60/79)]高于癌旁组织[51.9%(41/79)]及正常组织[13.9%(11/79)],差异均有统计学意义(P〈0.05或P〈0.01)。②ERK-2蛋白阳性表达率在不同年龄(〈60岁、≥60岁)[77.5%(31/40)、74.4%(29/39)]、性别(男/女)[70.7%(29/41)、68.4%(26/38)]、Dukes分期(A、B、C、D期)[73.3%(11/15)、76.7%(23/30)、71.4%(10/14)、70.0%(14/20)]、癌胚抗原水平(〈5μg/L、≥5μg/L)[68.2%(15/22)、64.9%(37/57)]因素间进行比较,差异均无统计学意义(P〉0.05);ERK-2蛋白阳性表达率在不同分化程度(高、中分化/低分化)[82.8%(48/58)、57.1%(12/21)]、浸润深度(未穿透肌层/浆膜或浆膜外)[46.2%(6/13)、72.7%(48/66)]和淋巴结转移(阳性/阴性)[76.5%(26/34)、53.3%(24/45)]因素间比较,差异有统计学意义(P〈0.05)。③ERK-2蛋白阳性表达患者12个月生存率(58.2%)与ERK-2蛋白阴性表达患者(60.4%)比较,差异无统计学意义(P〉0.05);ERK-2蛋白阳性表达患者24个月生存率(38.7%)低于ERK-2蛋白阴性表达患者(58.6%),差异有统计学意义(P〈0.05)。结论 ERK-2蛋白阳性表达与结肠癌多种临床病理因素有关,检测ERK-2蛋白对结肠癌的诊疗有一定意义。 Objective To investigate the expression and clinical significance of extracellular signal-regulated kinase-2 in colon cancer. Methods 79 patients with colon cancer confirmed by surgical pathology in People's Hospital of Linqu County from January 2008 to December 2010 were selected. Positive expression of ERK-2 protein in normal colon tissue, para-carcinoma tissue and colon cancer tissue were detected by immunohistochemical streptavidin-peroxidase (SP). Relationships between ERK-2 protein expression and clinicopathological characteristics, prognosis of colon cancer were analyzed. Results ①Positive expression rate of ERK-2 protein in colon cancer tissue [75.9% (60/79)] was higher than that in para-carcinoma tissue [51.9% (41/79)] and normal colon tissue [13.9% (11/79)], the differences were statistically significant (P 〈 0.05 or P 〈 0.01).②The ERK-2 protein positive expression rates in patients with different ages (〈60 years, ≥60 years) [77.5% (31/40), 74.4% (29/39)], gender (male/female) [70.7% (29/41), 68.4% (25/38)], Dukes stages (A, B, C, D) [73.3% (11/15), 76.7% (23/30), 71.4% (10/14), 70.0% (14/20)], CEA levels (〈5 μg/L,≥5 μg/L) [68.2% (15/22), 64.9% (37/57)] were not statistically significant (P 〉 0.05); the ERK-2 protein positive expression rates in patients with different elements of differentiated degrees (high, moderately differentiated/low differentiated) [82.8% (48/58), 57.1% (12/21)], depth of invasion (inside the muscularis/serous or serosa outside) [46.2% (6/13), 72.7% (48/66)], lymphatic metastasis (positive/negative) [76.5% (26/34), 53.3% (24/45)] were statistically significant (P 〈 0.05).③The difference of 12 months survival rate between patients with ERK-2 protein positive expression (58.2%) and patients with ERK-2 protein negative expression (60.4%) was not statistically significant (P 〉 0.05); 24 months survival rate in patients with ERK-2 protein positive expression (38.7%) was lower than those in patients with ERK-2 protein negative expression (58.6%), the difference was statistically significant (P 〈 0.05). Conclusion ERK-2 protein positive expression has relationship with variety of clinical pathological factors, ERK-2 protein detection has significance to diagnosis and treatment of colon cancer.
