骨科围手术期应用抗菌药物的干预研究

Intervention research of antibacterial drugs application during orthopaedic perioperative period

目的探讨某院骨科手术抗菌药物围手术期临床应用情况与干预措施的实施效果。方法抽取2009年3～9月外科手术患者270例作为对照组,另抽取2010年1～12月相似病种患者260例作为干预组,依据《抗菌药物临床应用指导原则》,比较2组手术感染率、平均住院天数、药费以及抗菌药物应用的合理性。结果该院实施合理用药方案后,降低了患者的药费,特别是抗菌药物费用,提高了术前预防用药的合理性,但对住院时间和手术切口感染率无影响,具有有效性和可行性。结论该院抗菌药物应用率偏高,各类手术预防用药指征不明确,用药目的和针对性不强,但通过干预取得了预期的效果。

Objective To investigate the clinical application situation and implementation effect of intervention meas- ures of antibacterial drugs application during orthopaedic perioperative period. Methods 270 surgery patients were selected as the control group from Mareh to September 2009 in a hospital, another 260 similar eases were selected January to December 2010, and investigated the perioperative use of prophylactic antibiotics based on the Guiding Principle of the Clinical Applica- tion of Antibaeterials. Compared the infection rate of operation, average hospitalization days, charges for medicine and rationality of antibiotics application of 2 groups. Results After the implementation of rational drug use, the cost of medicine was reduced, especially the cost of antibacterial drugs. And the rationality of preoperative preventive drug application was improved, but it had no effect to the duration of hospital stay and the operation incision infection rate, so the intervention measure is feasible and effective. Conclusion Drug application rate of antibacterial in the hospital is high, the prevent drug indication in all kinds of operation to is not clear, the purpose of using and the pertinence is not strong, but the expected results are achieved through intervention.
