伽马钉与动力髋螺钉治疗股骨粗隆间骨折的临床疗效对比分析

Comparative analysis of the clinical efficacy of Gamma nails and dynamic hip screw in treatment of intertrochanteric fractures

目的比较伽马钉(Gamma钉)与动力髋螺钉(DHS)治疗股骨粗隆间骨折的临床疗效。方法将本院收治的64例股骨粗隆间骨折患者随机分为Gamma钉组与DHS组,分别采用Gamma钉与DHS给予闭合复位内固定治疗,治疗完成后比较两组患者手术指标、并发症发生情况,采用Harris评分对临床疗效进行评价,并记录骨折愈合时间。结果 Gamma钉组患者手术时间、术中出血量均显著低于DHS组(P<0.05),两组患者治疗优良率、并发症发生率、骨折愈合时间均差异无统计学意义(P>0.05)。结论 Gamma钉与DHS治疗股骨粗隆间骨折疗效确切,Gamma钉在手术时间及手术出血方面具有一定优势,临床应根据患者情况进行治疗选择。

Objective To compare clinical efficacy of Gamma nail and dynamic hip screw（DHS） in treatment of intertrochanter ic fractures.Methods 64 cases of intertrochanteric fracture patients in our hospital were randomly divided into Gamma nail group and DHS group.Both groups were given closed reduction and internal fixation.Surgery indicators,compli cations of the two groups were compared after the treatment.The Harris hip score was used to evaluate the clinical ef ficacy and time of fracture healing was recorded.Results Mean operative time,blood loss volume of Gamma nail group were significantly lower than the DHS group（P0.05）,and there was no significantly difference between the two groups of patients in excellent rate,complication rate and fracture healing time（P0.05）.Conclusion Gamma nail and DHS both have exact effect in treatment of intertrochanteric fractures,and the Gamma nail has certain advantages in opera tive time and blood loss,and clinical selection should be based on the patient＇s condition.
