人工流产后生殖系统感染与不孕症关系的调查和干预对策的探讨

Investigation on the relationships between reproductive tract infection after abortion and infertility and exploration of the countermeasures

目的调查人工流产后生殖系统感染与不孕症的关系,并探讨干预对策。方法选取因不孕症前来就诊且有过人工流产史的患者120例为调查组,选取同时期前来就诊的无人工流产史的不孕症患者120例作为对照组,自行填写问卷或给予感染检查;调查人员对问卷内容进行统计学分析,各个病因所致的不孕症采用描述性分析,对人工流产后不孕症的高危因素进行Logistic回归分析,对人工流产后生殖系统感染与不孕症进行相关性分析。结果由输卵管因素造成的不孕是人工流产后不孕症发生的主要原因,盆腔炎史与由沙眼衣原体感染、解脲支原体感染造成的亚急性输卵管内膜炎是人工流产后不孕症发生的高危因素,而人工流产后生殖系统感染与不孕症的发生之间存在相关性。结论医疗机构需要对已婚及未婚女性增强生殖健康知识的宣教工作,降低人工流产后不孕症发生的高危因素,并有针对性地预防人工流产术后生殖系统的感染。

Objective To investigate the relationships between reproductive tract infection after abortion and infertility and explore the countermeasures.Methods 120 patients with history of induced abortion came to visit doctors due to infertility were selected as investigation group,while another 120 infertility patients without induced abortion were clas sified into control group.The questionnaires were filled and infection examinations were provided.After collected the questionnaire,its contents were statistically analyzed by investigators.The causes of infertility were given with descrip tive analysis,the high risk factors of infertility after induced abortion was given regression analysis,and the correlation analysis was applied between reproductive tract infection after abortion and infertility.Results Infertility due to fallopian tube was the main cause leaded to occurrence of infertility after abortion.The pelvic inflammation history and suba cute endosalpingitis caused by chlamydia trachomatis and mycoplasma urealytium were two high risk factors which triggered the infertility after abortion.There was correlation between reproductive tract infection after abortion and in fertility.Conclusion In order to reduce the high risk factors of infertility after abortion and take targeted prevention of the reproductive tract infection after abortion,the enhancing propaganda and education of reproductive health for mar ried and unmarried female is essential by medical institutions.
