摘要
目的初步研究慢性溃疡性结肠炎(UC)的证候特点。方法通过临床调查,运用聚类分析及主成分分析,研究慢性溃疡性结肠炎的中医辨证规律。结果 1.通过证型聚类,可以将慢性溃疡性结肠炎证型分为气虚湿热证、脾肾阳虚证、气滞血瘀证和阴虚血瘀证四大类。2.慢性溃疡性结肠炎各证型的结构为气虚:神疲懒言、食少、倦怠乏力、面色萎黄;湿热:经常腹泻、大便脓血、里急后重、肛门灼热、苔腻;气滞血瘀:情志抑郁、腹胀、舌紫暗、舌有斑点、月经推迟或延长;阴虚:经常便秘、五心烦热、盗汗、舌苔少、脉细;脾肾阳虚:面色白光白、腹痛喜按、形寒肢冷、舌淡。结论运用聚类分析及主成分分析对慢性溃疡性结肠炎进行中医辨证证型研究,分析其证型、证型结构,为进一步探讨本病辨证规律及立法、组方、用药规律打下了基础。
Objective To primarily explore the TCM syndrome characteristics of chronic ulcer ative colitis(CUC).Methods Cluster Analysis and principal component analysis methods were ap plied to explore CUC TCM differentiation rules through clinical investigation.Results 1) Through Cluster Analysis,CUC could be divided into Qi-deficiency and damp-heat syndrome,Yang-defi ciency of spleen and kidney syndrome,Qi stagnation and blood stasis syndrome and Yin deficiency and blood stasis syndrome by syndrome classification.2) Syndromes of CUC included that: Qi-defi ciency performed by lassitude and no desire to speak,eating less and fa tigue,dull complexion;damp-heat performed by diarrhea,dysentery with blood and pus,tenesmus,anal burning and tough tongue with thick or greasy fur;Qi stagnation and blood stasis performed by depression,abdominal distending,cyanotic tongue,delayed menstruation or metrorrhagia;Yin deficiency performed by constipation,fanre feeling,night sweet,less fur on tongue and weak pulse;Yang deficiency of spleen and kidney manifested by flushed complexion,abdomen pain desiring pressing,cold limbs and tender tongue.Conclusion Applying Cluster Analysis and principal component analysis to observe CUC syndromes and their structure can offer a base for further researches on rules of CUC differentiation,treatment,constructing prescription and choosing medicine with the goal of regulating CUC syndrome differentiation and treatment.
