摘要
目的观察大黄治疗急性重度敌敌畏中毒合并酒精中毒所致心肌损害的临床疗效。方法将70例急性重度敌敌畏中毒合并酒精中毒患者随机分为对照组与治疗组,每组35人。两组患者均于24 h内抽取空腹静脉血,测定肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(HBDH)、天门冬氨酸氨基转移酶(AST),所有患者均描记心电图、行心电监测。对照组给予常规综合治疗,治疗组在常规治疗基础上,每日给予大黄液100 mL 2次口服或鼻饲,应用5 d。观察两组患者心肌酶恢复正常时间、住院期间心律失常发生情况。结果两组患者在入院时其心肌受损、心律失常发生情况差异无统计学意义(P>0.05)。治疗组经应用大黄治疗后,其心肌酶恢复正常时间、恶性心律失常发生率明显低于对照组(P<0.05)。结论应用大黄救治急性重度敌敌畏中毒合并酒精中毒所致心肌损害具有较好的疗效,值得临床推广应用。
Objective To observe the curative efficacy of using Dahuang in treating myocardial damage of severe acute dichlorvos poisoning affiliated with acute alcoholism. Method 70 cases of severe acute dichlorvos poisoning affiliated with acute alcoholism were divided into two groups, every group consisted of 35 cases. Fasting venous blood of all patients were collected within 24 hours of admission to determine serum creatine kinase(CK),lactate dehydrogenase(LDH),ct-hydroxybutyrate dehydrogenase(HBDH),aspartate aminotransferase(AST), the surface electrocardiogram and electrocardiogram monitoring were recorded.The Control group were treated with routine comprehensive treatment. On the basis of control group, the treatment group were used Dahuang liquid (100ml, two times a day) through oral or nasogastric feeding, application for 5 days. The time of myocardial enzyme returned to normal and occurrence of cardiac arrhythmia was compared. Results There was no significant differernce between two groups in myocardial damage and occurrence of cardiac arrhythmia at the time of admission (P〉0.05).After the patients of treatment group were treated with Dahuang, the Myocardial enzyme returned to normal time and incidence of malignant arrhythmia were significantly lower than that of the control group (P〈0.05). Conclusions For the cases of severe acute dichlorvos poisoning affiliated with acute alcoholism, using Dahuang has a better curative effect for myocardial damage, which deserves further clinical application..
