人工鼻预防危重患者全麻术后肺部感染的效果分析

Effect of heal and moisture exchanger on prevention of pulmonary infections in critical patients after general anesthesia

目的探讨人工鼻预防危重患者全麻术后肺部感染的效果,为预防控制术后肺部感染提供参考。方法选取2011年1-12月全麻术后危重患者128例,将其分为两组,对照组60例,为2011年1-6月未使用人工鼻术后危重患者,人工鼻组68例,为2011年7-12月使用人工鼻术后危重患者。结果人工鼻组68例患者中有6例发生肺部感染,感染率为8.8%,对照组60例患者中有15例发生肺部感染,感染率为25.0%,两组差异有统计学意义(P<0.05);人工鼻组患者术后几乎不需要冲洗和换水,两组患者每日护理项目和所耗护理时间比较,差异有统计学意义(P<0.05)。结论人工鼻可明显减少术后危重患者肺部感染的发生。

OBJECTIVE To explore effect of the heal and moisture exchanger（HME）on prevention of pulmonary infections of the critical patients after the general anesthesia operation so as to provide basis for the prevention and control of postoperative pulmonary infections.METHODS A total of 128critical patients who underwent the general anesthesia in the hospital from Jan to Dec 2011were selected as the research objects and were divided into two groups,the 60patients who did not use HME from Jan 2011to Jun 2011were set as the control group,and the 68patients who used HME from Jul 2011to Dec 2011were set as the HEM group.RESULTS Of 68patients in the HEM group,there were 6（8.8%）cases with the pulmonary infections;of 60cases in the control group,there were 15（25.0%）cases with the pulmonary infections,the difference was statistically significant（P 0.05）.The patients of the HME group almost didn＇t need to flush or change water,and the difference in the nursing items or the nursing time between the two groups was statistically significant（P〈0.05）.CONCLUSION The HME can remarkably reduce the incidence of postoperative pulmonary infections in the critical patients.
