小剂量脾多肽对化疗后血小板计数恢复过程的影响 被引量:11

摘要 目的观察脾多肽治疗化疗引起的血小板减少的疗效和不良反应。方法化疗结束后隔天复查血常规,将血小板数低于75×109/L的病例58例随机分到2组。治疗组应用脾多肽2 ml+0.9%生理盐水100 ml,静脉滴注,每天1次,血小板计数>100×109/L后停药。对照组不用脾多肽,不用其他任何升血小板药物,观察等待血小板数自行恢复。结果治疗组血小板恢复至≥75×109/L和≥100×109/L的平均时间为(4.73±1.46)d与(6.27±1.53)d,明显短于对照组的(8.64±2.67)d与(14.64±2.38)d(P<0.05)。治疗后(PLT<75×109/L后)的血小板计数第3、7、15 d分别为(64.1±21.83)×l09/L、(105.77±18.36)×l09/L和(179.27±75.35)×l09/L,明显高于对照组的(50.36±16.17)×l09/L、(72.75±12.36)×l09/L和(106.07±16.73)×l09/L。治疗组中不良反应为:发热(发生率6.7%)。结论应用小剂量脾多肽治疗恶性肿瘤化疗后引起的血小板减少有效缩短了血小板恢复的时间,提高了化疗后血小板的最低值,减少了需输注血小板的量,不良反应可耐受。为化疗引起的血小板减少的防治增加了治疗方法。 Objective To observe the curative effect and adverse reaction of lienal polypeptides for the treatment of chemotherapy in- duced platelet reduction. Methods Every day a review of blood after chemotherapy was made. Accordingly, 58 patients whose platelet count was less than 75×109/L were randomly divided into two groups: the treatment group used lienal polypeptides 2 m1±0.9% normal saline, intra- venous drip, once a day, and stoped at the platelet count after 〉100 xl09/L. The control group was without lienal polypeptides or any other drugs to increase the platelet count, and the result of platelet count recovery was observed. Results The average time of treatment group platelet recovery to ≥75×109/L and ≥ 100x 109/L was (4.73±1.46)d and (6.27±1.53)d,significantly shorter than that of the control group, which was (8.64±2.67)d and (14.64±2.38)d (P 〈0.05). After treatment (PLT 〈 75×109/L) the platelet count on 3 rd, 7 th, 15 th day was ( 64.1 ± 21.83 )x 109/L, ( 105.77 ± 18.36 ) x 109/L, and ( 179.27 ± 75.35 ) x 109/L, respectively, higher than that in control group, which was (50.36 ± 16.17)×109/L, (72.75 ± 12.36)×109/L, and( 106.07 ± 16.73)×109/L respectively. Adverse reaction in the treatment group was fever (incidence 6.7%). Conclusion A small dose of lienal polypeptides are effective for thrombocytopenia after chemotherapy to treat malignant tumor, which may reduce the recovery time of platelet count, increase the lowest platelet count after chemotherapy, and reduce the platelet transfusion vol- ume. The adverse reactions can also be tolerated. A small dose of lienal polypeptides can increase the treating method for chemotherapy-induced thrombocytopenia.
