摘要
目的探讨2型糖尿病患者血正五聚蛋白3(PTX3)、尿αl微球蛋白(αlMG)与免疫球蛋白G(IgG)在糖尿病肾病早期诊断中的意义。方法 2型糖尿病患者85例,根据尿白蛋白排泄率(UAER)分成正常蛋白尿组、微量蛋白尿组与临床蛋白尿组,分别检测三组PTX3、αlMG与IgG。结果微量白蛋白尿组PTX3、αlMG与IgG水平显著高于正常白蛋白尿组,差异有统计学意义(P<0.05)。临床白蛋白尿组PTX3、αlMG与IgG水平显著高于正常白蛋白尿组与微量白蛋白尿组,差异有统计学意义(P<0.05)。相关性分析显示,PTX3与尿微量白蛋白(mAlb)、αlMG、IgG均呈正相关(P<0.01)。结论 PTX3、αlMG与IgG水平的检测对2型糖尿病患者早期肾损害具有诊断价值,单独mAlb检测诊断糖尿病肾病存在局限性。
Objective To investigate the clinical value of blood Pentraxin-3(PTX3),urinary α1-microglobulin(α1MG) and urinary immunoglobin G(IgG) in type 2 diabetic patients with diabetic nephropathy.Methods According to UAER,85 patients with type 2 diabetes were divided into 3 groups called normoalbuminuria,microalbuminuria and macroalbuminuria respectively.PTX3,α1MG and IgG were measured.Results PTX3,αlMG and IgG in microalbuminuria was significantly higher than those in normoalbuminuria(P0.05).PTX3,αlMG and IgG in macroalbuminuria were significantly higher than those in normoalbuminuria and microalbuminuria(P0.05).Correlation analysis showed PTX3 was positively correlated with mAlb,αlMG and IgG(P0.01).Conclusions The detection of levels of PTX3,αlMG and IgG have diagnostic value for the early renal injury in type 2 diabetic patients.Diabetic kidney disease can not be detected by microalbuminuria in some cases.
