地佐辛复合芬太尼在胸科手术术后静脉镇痛中的临床观察

Effects of dezocine combined with fentanyl on patient-controlled intravenous analgesia after thoracic surgery

摘要 目的观察、评价地佐辛复合芬太尼在开胸手术患者术后自控静脉镇痛的临床效果。方法选择在粤北人民医院行择期开胸手术患者80例,ASA分级为Ⅰ~Ⅱ级,将80例入选患者随机分为地佐辛复合芬太尼自控静脉镇痛(PCIA)组(DF组)和芬太尼自控静脉镇痛组(F组),每组40例,实施全凭静脉麻醉。术后患者均行PCIA,PCIA配方:DF组100 ml药液含地佐辛0.4 mg/kg,芬太尼10μg/kg,F组100 ml药液含芬太尼20μg/kg。记录两组患者术后4、8、12、24、48 h镇痛评分、镇静评分及不良反应。结果两组在术后各时间点的疼痛VAS评分、镇静Ramsay评分比较差异均无统计学意义(P均>0.05)。DF组术后各时间点的芬太尼累积消耗量均显著少于F组(P均<0.01)。F组的总不良反应发生率为47.3%,DF组为8.7%(P<0.05)。结论地佐辛联合芬太尼应用于胸科术后PCIA镇痛效果确切可靠,可减少芬太尼用量,患者术后的总体满意度高且不良反应发生率低。 Objective To evaluate the effect of dezocine combined with fentanyl on patient-controlled intravenous analgesia(PCIA) after thoracic surgery.Methods Eighty patients admitted in Yuebei People's Hospital hospital with American Society of Anesthesiologists(ASA)Ⅰ-Ⅱ were scheduled for thoracic surgery and randomly divided into fentanyl group(group F)and dezocine combined with fentanyl group(group DF).There were 40 patients in each group.Intravenous anesthesia with tracheal intubation and PCIA were performed in all the patients.In group DF,dezocine 0.4 mg/kg and fentany 10 μg/kg with 0.9% normal saline diluted to 100 ml;and in group F,fentanyl 20 μg/kg with 0.9%normal saline diluted to 100 ml.Visual analogue scale(VAS),Ramsay scores and adverse reactions were recorded at 4,8,12,24 and 48 h after operation.Results There was no significant difference in VAS score or Ramsay score(P0.05).Compared with group F,group DF had less consumption of fentanyl at each time point(P0.01).The total incidence rates of adverse reactions in group F and group DF were 47.3%and 8.7%.(P0.05).Conclusion PCIA with dezocine combined with fentanyl is an effective method for the postoperative analgesia in thoracic surgery.It can reduce the consumption of fentanyl and have high patient satisfaction and low incidence of adverse reactions.
