肿瘤相关糖蛋白TAG-72在胃癌及其癌前组织中的表达

Expression of tumor-associated glycoprotein (TAG-72) in gastric carcinoma and its precancerous lesions

应用单克隆抗体B72.3和免疫酶标方法检测了肿瘤相关糖蛋白TAG-72在110例胃癌,36例肠型不典型增生和303例肠化组织中的表达。根据粘液染色将肠化分为3型:Ⅰ型为完全型,Ⅱ和Ⅲ型为不完全型,Ⅱ、Ⅲ两型的区别在于后者柱状粘液细胞含有硫酸粘液,而前者则无硫酸粘液。胃癌TAG-72染色阳性率为60.9％。肠型不典型增生及癌旁肠化组TAG-72阳性率显著高于其它良性病变肠化组(P<0.05～0.01);Ⅲ型肠化TAG-72阳性率显著高于Ⅰ、Ⅱ型肠化(P<0.01),提示Ⅲ型肠化与胃癌发生有关。结果表明,检测胃病组织TAG-72有助于印戒细胞癌的诊断和鉴别诊断,对确定胃癌高危人群,提高早癌的检出率亦可能有重要意义。

The expression of tumor-associated glycoprotein (TAG-72) identified by monoclonal antibody B72.3 was investigated in HO resected specimens of gastric carcinoma,36 biopsied specimens of dysplasia,and 303 specimens of intestinal metaplasia(IM).IM was subdivided into complete Type I and incomplete Types Ⅱ and Ⅲ based on whether sulphomucin is present or absent in the mucin secreting columnar cells.The positive rate of TAG-72 in gastric carcinoma was 60.9%,and it was significantly higher in IM specimens with gastric carcinoma than in those with benign gastric lesions (P<0.05~0.001).The positive rate of TAG-72 was significantly higher in Type Ⅲ IM than in Types Ⅰ and Ⅱ IM (P<0.01),which suggests the close relationship between Type Ⅲ IM and gastric carcinoma.It is believed that TAG-72 is a useful tumor marker in the diagnosis of signet ring carcinoma,in the defining of the high-risk group,and in the detection of early cases of gastric carcinoma.
