摘要 目的探讨白细胞介素-2(IL-2)与白细胞介素-6(IL-6)在老年"隐匿性压疮"中的作用及表达,为临床老年压疮早期预防提供理论依据。方法将SPF级雄性老年SD大鼠12只随机分成对照组和模型组,均行10%水合氟醛麻醉后,对照组不受压,模型组采用自制的加压装置对大鼠股骨大转子及胫骨前区皮肤进行施压,2 h后两组大鼠一起实施安乐死。实验终点,观察受压皮肤肌肉组织病理学变化;检测10%匀浆肌组织中IL-2、IL-6和髓过氧化物酶(MPO)的含量。将各数据之间进行Pearson相关分析。结果模型组大鼠皮肤虽完整,但受压局部均出现持续非苍白性发红现象。光镜下模型组大鼠复层鳞状上皮变薄,结构层次欠清晰,表皮与真皮部分分离;肌纤维有断裂迹象,轻度水肿,间隙增宽;真皮和肌肉组织中均可见明显炎性细胞浸润。对照组与模型组比较,IL-2、IL-6、MPO均显著增高(P<0.01)。Pearson相关分析显示IL-2、IL-6和MPO的含量之间均呈显著正相关(r分别为0.809、0.844和0.930,P<0.05或P<0.01)。结论 IL-2、IL-6介导的炎症性反应在老年大鼠"隐匿性压疮"形成过程中起着重要作用。 Objective To explore the effects and expression of interleukin 2(IL-2) and interleukin 6(IL-6) on latent pressure sore of the senior,and to provide theoretical basis for the early clinical prevention of bedsore in the senior.Methods Twelve SPF-level senile male SD rats were randomly divided into control group and model group.After all the rats were narcotized with 10% hydration fluorine aldehyde,those in the control group were given no pressure,while those in the model group were given pressure on the skin in the anterior region of rats’ femur big rotor and tibia by a self-made pressure device.Both groups of rats received the euthanasia after 2 hours.At the end of the experiment,the pathological changes of the skin and muscle tissues of in the pressure region were observed,and detection was made in the content of IL-2,IL-6,and myeloperoxidase(MPO) in 10% homogenate of the muscle tissues.Then the Pearson correlation analysis was conducted to analyze those data.Results Although the skin of pressure region in the model group was complete,continuous redness was observed.Under the light microscope,the stratified squamous epithelium of rats in the model group became thinner;its structure layer lacked clarity;the epidermis and the dermis separated;the muscle fiber was found disrupted;mildly edematous and gap broadening occurred;inflammatory cell infiltration could be observed obviously in dermal and muscle tissues.Compared with the parameters of the model group,the IL-2,IL-6,and MPO in the control group increased significantly(P &lt; 0.01).Pearson correlation analysis indicated that there was a positive correlation between IL-2 and IL-6,between IL-2 and MPO,and between IL-6 and MPO(r = 0.809,0.844,and 0.930,P &lt; 0.05 or P &lt; 0.01).Conclusion Inflammatory reaction mediated by IL-2 and IL-6 plays an important role in the development of senile rats’ latent pressure sore.
