硫化氢后适应对大鼠心肌缺血-再灌注损伤后心功能的影响 被引量:3

摘要 目的探讨硫化氢后适应对大鼠心肌缺血-再灌注损伤的保护作用。方法采用结扎大鼠左冠状动脉前降支30 min,再灌60-120 min的方法制备在体心肌缺血-再灌注损伤模型。Weistar大鼠(n=24)随机分为3组(n=8):假手术组(Sham),缺血再灌注组(I/R)和H2S后适应组(H2S)。Sham组为仅穿线,不做结扎;I/R组为先缺血30 min,再灌120 min;H2S后适应组为缺血30 min后,在再灌注之前经股静脉注射NaHS(15μmol/kg)。记录缺血及再灌注时心率、再灌注性心律失常、左室收缩末期压(LVESP)及左室舒张末期压(LVEDP)及左室内压上升/下降最大速率(±dp/dtmax,mmHg/s)等心功能参数,并计算左室发展压(LVDP,LVDP=LVESP-LVEDP)。应用伊文氏蓝和TTC染色测定心肌梗死面积(%)。结果 H2S后适应组与I/R组降低了再灌注室性心律失常的发生率、持续时间及心律失常评分(P〈0.05),使心肌梗死面积减小(P〈0.05);同时,心功能参数得到明显改善(P〈0.01)。结论 H2S后适应能够减轻大鼠心肌缺血再灌注损伤,其中在拮抗再灌注性心律失常和恢复大鼠心功能方面作用明显。 【Objective】 To evaluate the effect of hydrogen sulfide postconditioning on myocardial ischemia-reperfusion injury in rats.【Methods】 The left anterior descending coronary artery was ligated for 30 min followed by a 180 min reperfusion in vivo.Wistar rats(n =24) were randomly divided into 3 groups(n =8).The sham group(Sham) was subjected to only threading but no ligation,ischemia-reperfusion group(I/R) to ischemia for 30 min and reperfusion for 120 min,and H2S postconditioning group(H2S) to 30 min of ischemia,then reperfused with NaHS(15 μmol/kg).Heart rate,reperfusion arrhythmias,left ventricular end-systolic pressure(LVESP),left ventricular end-diastolic pressure(LVEDP),left ventricular pressure rise/fall maximum rate(±dp/dtmax,mmHg/s) and left ventricular developed pressure(LVDP,LVDP=LVESP-LVEDP) were recorded and calculated.The myocardial infarct size(%) was measured by the application of Evans blue and TTC staining.【Results】 Incidence of ventricular fibrillation,its duration,arrhythmia score and myocardial infarct size were significantly lower in the H2S postconditioning group and I/R group(P 0.05),while the cardiac function parameters was improved significantly(P 0.01).【Conclusion】 H2S postconditioning could significantly reduce myocardial ischemia-reperfusion injury.
