摘要 目的探讨超声检查在类风湿性关节炎膝关节病变诊断中的应用价值。方法选择类风湿性关节炎并伴有膝关节病变患者42例,运用超声检测膝关节滑膜厚度、血流变化及血管增殖、关节积液、软骨及骨质破坏等情况,并做治疗前后对比观察。结果超声能够显示类风湿性膝关节炎患者关节囊积液;滑膜增厚,腔面毛糙不平,部分可见绒毛状突起;滑膜血管增生,为低速血流信号,多数为点状、短杆状,少数为树枝状血流信号;软骨下骨侵蚀。治疗后膝关节自觉疼痛、压痛降低。治疗前髌上囊液体厚度为(5.37±3.52)mm,滑膜厚度为(4.68±1.94)mm,滑膜动脉阻力指数为(0.60±0.05)。治疗后髌上囊积液、滑膜厚度及RI值差异有统计学意义(P<0.01)。结论超声能够敏感而准确地显示类风湿性膝关节炎患者滑膜炎症,血管翳形成,软骨下骨侵蚀等病理改变,并可用于类风湿性关节炎临床疗效客观评估。 Objective To investigate the efficacy of ultrasound (US) for evaluating rheumatoid arthritis of the knee. Methods Forty-two patients with rheumatoid arthritis underwent US examinations of the knees before and after treatment. The synovial membrane thickness, changes in local blood flow and vascular proliferation, joint effusion, cartilage and bone destruction were compared before and after treatment. Results Ultrasound demonstrated joint effusion, synovial thickening, surface irregularity with villous projections. Synovial angiogenesis with low blood flow, punctate, short rod-like or dendritic flow signals as well as suhchondral bone erosions was also observed. Joint pain and tenderness resolved after treatment. There was significant (P 〈 0.01) decrease in the depth of joint effusions, synovial membrane thickness, and resistance index before (5.37 ± 3.52 mm, 4.68 ± 1.94 mm, 0.60 ± 0.05) and after treatment. Conclusions Ultrasound is sensitive and accurate for detecting pannus formation and subchondral erosion. US can be used to monitor the treatment effect of rheumatoid arthritis of knee.
