摘要
目的对广西男性吸毒人群包皮易感HIV的免疫生物学因素进行研究,为包皮环切术预防男性吸毒人群通过生殖道感染HIV提供生物学依据。方法采用HE染色法、免疫组化Super PicTure法测定研究对象包皮角质化的程度以及HIV靶细胞的成分和密度。结果研究对象包皮标本内、外板的角质层平均厚度分别为1.37±0.549和3.23±1.178,差异有统计学意义(P=0.000)。研究对象HIV靶细胞抗原CD1a和HLA-DR在内包皮的平均密度分别为3.06±0.800和10.20±1.860,在外包皮的平均密度分别为1.65±0.787,9.25±1.437。两者在内板上的平均密度均高于外板上的平均密度。不同年龄、民族、吸毒人群和普通人群间,HIV靶细胞在包皮内外板上的平均密度差异无统计学意义。结论成年男性菲薄的内包皮和包皮上密度不同的靶细胞是HIV进入男性生殖道的主要门户,这是男性行包皮环切术后可减少HIV感染机会的可能原因,也是包皮环切可预防男性吸毒人群通过性途径感染HIV的生物学基础。
Objective To study the immunological and biological factors that effect the susceptibility of male foreskin to HIV infection in Guangxi injection drug user (IDUS), and to provide biological data for circumcision preventing HIV invaded IDUs. Methods Keratin of inner and outer foreskin of subjects was detected with hematoxylin-eosin staining method. By immunohistochemical technique Super PicTure method (one step method) the HIV target cells were detected for locating and measuring the density. Results The average thickness of kera- tin on inner and outer foreskin among all subjects were (1.37 ±0. 549) and (3.23 ± 1. 178) respectively, the difference was statistically significant (P = 0. 000). In all subjects, the average density of HIV target cell antigens CDla and HLA-DR in inner foreskin were (3.06 ±0. 800) and ( 10.20 ± 1. 860), and in outer foreskin the average density were ( 1.65 ± O. 787) and (9.25 ± 1. 437). Comparing the average density of these target cells, it was found that the density of Langerhans cells CD1 a antigen and Macrophage HLA-DR antigen in inner foreskin were higher than those in the outer foreskin. Comparing the average density of these HIV target cells between different groups, ages, Han, Zhuang male, it was found that there were no statistically significant differences of the density of HIV target cell antigens, in inner foreskin. Conclusion The meager inner foreskin and the different HIV target cells are the main portal that HIV invades adult males, which may be the main reason that male circumcision can protect against HIV infection, and the biological basis that male circumcision can protect against HIV infection through sexual behavior in IDUs.
