慢性乙型肝炎病毒感染者肝细胞膜HLA—Ⅱ类抗原表达与病毒复制

Expression of HLA class II antigens on hepatocyte membrane in chronic hepatitis B virus carriers

用免疫组织化学法观察49例慢性乙型肝炎病毒(HBY)感染者肝细胞膜HLA—Ⅱ类抗原表达。发现血清HBeAg(+)者23例中13例,抗HDe(+)者26例中仅4例肝细胞膜表达HLA-Ⅱ类抗原(P<0.01):肝内HBcAZ(+)者17例中有11例,肝内HBcAg(-)者32例中仅6例肝细胞膜表达HLA-Ⅱ类抗原(P<0.05)。说明慢性HBV感染者肝细胞膜HLA-Ⅱ类抗原表达与病毒复制有关,与肝病变活动性无关。

In order to study the relationship between replicative status and HLA antigens, HLA class II antigen on hepatocyte membrane was analyzed in the liver biopsies from 49 patients with chronic hepatitis B virus infection. The results revealed that expression of IILA class II antigen on hepatocyte membrane was in 57% ( 13/23 ) of HBeAg positive, only in 15% ( 4/26 ) of anti-HBe positive chronic HBV carriers. When compared with intrahepatic HBcAg, the display of HLA class II antigen on hepatocyte membrane was found in 65% positive and in 19% negative patients. There was no significant difference between minor hepatic diseases and active liver diseases. Our findings suggested that the display of HLA class II antigen on hepatocyte membrane might reflect viral replication in the chronic HBV carriers.
