普米克令舒联合万托林吸入在大容量全肺灌洗治疗尘肺中的作用

Effect of inhalation of pulmicort respules combined with vantorin in the process of pneumoconiosis massive whole-lung lavage

目的评价普米克令舒联合万托林吸入在大容量全肺灌洗治疗尘肺中的应用价值。方法单侧分期大容量全肺灌洗(WLL)患者90例,45例行常规大容量全肺灌洗,45例治疗组患者自术前3天～2次大容量全肺灌洗结束后3天予普米克令舒联合万托林雾化吸入。比较两组术中并发症、术后肺部灌洗液残余量差异及术后肺功能改善和临床症状综合评分变化。结果与常规WLL组相比,治疗组患者术中发生气道痉挛、低氧血症、气道压增高患者例数减少(P<0.01);灌洗液残余量减少(P<0.05);治疗后两组肺功能及临床症状均较术前改善,以治疗组改善更为显著(P<0.05)。结论普米克令舒联合万托林雾化吸入能有效减少尘肺患者WLL并发症,减少灌洗液残余量,提高灌洗效果,加快术后恢复。

Objective To evaluate the significance of inhalation of pulmicort respules combined with vantorin in the process of pneumoconiosis massive whole-lung lavage.Methods 90 patients with pneumoconiosis treated with unilateral stage high-capacity whole lung lavage were selected and evenly divided into the whole-lung lavage（WLL） group and the treatment group randomly.The treatment group was also administrated by inhalation of pulmicort combined vantorin from three days before WLL to three days after WLL.Then the therapeutic effect of the two groups was evaluated by WLL complications,the remanent fluids,lung function test and the improvement of clinical symptoms.Results The occurrence of intraoperative airway spasm,hypoxemia,airway pressure increased were lower in the treatment group than that in the WLL group（P 0.01）.Lung function and clinical symptoms were better improved in the two groups after the treatment,and the improvement was more pronounced in the treatment group than in the WLL group（P〈0.05）.Conclusion The inhalation of pulmicort respules combined with vantorin can reduce complications and remanent fluids,improve lavage effect and accelerate their postoperative recovery in the treatment of patients with pneumoconioses treated with massive whole lung lavage.
