经纤支镜介入冷冻治疗支气管结核的疗效分析

Curative effect analysis of interventional therapy for bronchial tuberculosis with cryotherapy via fibre bronchoscope

目的探讨纤支镜下冷冻治疗支气管结核的临床效果。方法对70例溃疡坏死型、肉芽增殖型及两者混合型的支气管结核患者分为治疗组和对照组。两组均采用正规抗结核化疗HRZE。治疗组在抗结核化疗的基础上加用纤支镜介入低温冷冻治疗,每周一次,连续3～8次,观察两月。结果治疗两月后痰菌阴转率对照组和治疗组分别为66.7%和90.6%(P<0.01);影像学检查显示病灶吸收率对照组和治疗组分别为64.5%和92.3%(P<0.01);支气管镜检查显示治疗有效率对照组和治疗组分别为70%和94.9%(P<0.01)。结论全身化疗联合支气管镜介入冷冻治疗支气管结核优于单纯抗结核化疗。

Objective To observe and evaluate the clinical curative effect of interventional therapy for bronchial tubeerculosis by cryotherapy via fibre bronchoscope.Methods 70 cases have been selected and divided into treated group,treated by interventional therapy with fibre bronchoscope combined with anti-tuberculosis chemotherapy,and control group,treated with anti-tuberculosis chemotherapy.The curative effects were observed two months later.Results After two months,the sputum negative conversion rates were 66.7% for treated grounp,and 90.6% for control grounp（P〈0.01）;the lesions absorption rates by X ray of detection were 64.5% for treated grounp,and 92.3% for control grounp（P〈0.01）;the recovery rates observed by bronchoscopy were 70% for treated grounp,and 94.9% for control grounp（P〈0.01）.Conclusion The clinical curative effect of interventional therapy by cryotherapy via fibre bronchoscope combined with anti-tuberculosis chemotherapy is superior to single chemotherapy for bronchial tuberculosis.
