摘要
目的介绍一种人工晶状体全脱位手术的新方法。方法人工晶状体全脱位7例(7眼)。预置23G灌注,采用新型长虹膜拉钩通过透明角膜,直接钩出脱位于玻璃体腔的人工晶状体进入前房,再行原人工晶状体睫状沟缝线固定术。观察术前、术后视力及并发症情况。结果3眼发生角膜水肿,1眼发生高眼压;4眼视力达到人工晶状体脱位前最佳矫正视力,2眼视力较脱位后视力提高2行以上,1眼视力无明显变化;无严重并发症。结论应用长虹膜拉钩(50mm),使人工晶状体全脱位手术操作简化,眼内组织损伤较轻,手术时间缩短,效率高,并发症少。
Objective To introduce a kind of new surgical method for intraocular lens (IOL) dis- location. Methods Intraocular lens dislocation in 7 cases (7 eyes) were involved. With preset 23 G perfu- sion, new type long iris hook stabbed through clear cornea, to catch the IOL from the vitreous cavity and put it into the anterior chamber, and fix the IOL by ciliary sulcus suture. The preoperative and postoperative vis- ual acuity and complications were observed. Results Corneal edema occurred in 3 eyes, high intraocular pressure in 1 eye. The postoperative visual acuity reached the best corrected visual acuity before IOL disloca- tion in 4 eyes, improved more than two lines in 2 eyes, and no obvious change in 1 eye. No serious compli- cation observed in the 7 cases. Conclusion Application of long iris hook (50 mm) can simplify the opera- tion for IOL dislocation, reduce tissue damage, shorten operation time, with high efficiency and less compli- cation.
