早期玻璃体切除治疗白内障术后感染性眼内炎

Early vitrectomy in the treatment of infectious endophthalmitis after cataract surgery

目的探讨早期玻璃体切除手术治疗白内障术后眼内炎的临床疗效。方法分析石首市人民医院2006年1月至2012年12月白内障手术后感染性眼内炎行玻璃体切除手术者12例的临床资料,将发病后3 d内行玻璃体手术的患者与3 d后手术患者进行比较。结果所有发病后3 d内行玻璃体切除者术后视力均好于术后第2天视力,无一例患者需玻璃体腔填充硅油,3 d后行玻璃体手术者均需玻璃体填充硅油,术后最佳矫正视力均低于术后第2天视力。结论白内障术后眼内眼一旦确诊,在玻璃体腔注药无效时应马上行玻璃体切除手术,可以最大限度保留患者的视力。

Objective To investigate the effect of early vitrectomy on infectious endophthalmitis after cataract surgery.Methods The clinical data of 12 cases of infectious endophthalmitis after cataract surgery from January 2006 to December 2012 were analyzed,patients who had the vitrectomy within 3 days of infection and the patients who had vitrectomy after 3 days of the infection was compared.Results All the cases that received vitrectomy within 3 days of infection presented better invision,while none of them needed silicone oil tamponade.But those who received vitrectomy after 3 days all needed silicone oil tamponade,and all the best corrected vision after operation were lower than the vision of the second day after cataract surgery.Conclusion Once the infectious endophthalmitis is diagnosed and the intraocular injection is useless,then the vitrectomy must be taken immediately,which may best preserve the vision of the patients.
