不同透析模式对改善尿毒症患者顽固性皮肤瘙痒的效果观察 被引量:2

摘要 目的:探讨不同透析模式对改善尿毒症患者顽固性皮肤瘙痒的效果观察。方法:选择尿毒症顽固性皮肤瘙痒患者50例,将其随机分观察组和对照组各25例,观察组患者采取血液透析与血液透析滤过联合应用,对照组仅实施血液透析。治疗3个月后检测血肌酐、血清尿素氮、甲状旁腺激素和血β2微球蛋白,采用视觉模拟评分法评估瘙痒程度。结果:观察组患者治疗3个月后血中血肌酐、血清尿素氮、血β2微球蛋白、甲状旁腺激素水平均低于对照组(P<0.05),皮肤瘙痒程度轻于对照组(P<0.05)。结论:血液透析联合血液透析滤过治疗可优势互补,能够更有效清除患者的代谢产物、致病因子等物质,明显减轻尿毒症患者顽固性皮肤瘙痒症状。 Objective:To investigate the effect of hemodialysis and hemodiafiltration on improving uremic pruritus.Methods:50 patients with uremic pruritus were randomly divided into observation group(HDF+HD) and control group(group HD).After treatment 3 months,to detect serum creatinine(Scr),blood urea nitrogen(BUN),parathyroid hormone(PTH) and serum β2 microglobulin(β2-MG),and assess pruritus degreel by visual analogue scale.Results:After treatment 3 months,the blood Scr,BUN,β2-MG,PTH levels had significant difference of two groups,but the effect of observation group was better than control group.The pruritus score in observation group was lower than the control group,the difference was statistically significant(P0.05).Conclusion:The treatment mode of HDF+HD through two different blood purification methods complement each other,can more effectively remove metabolites,patients with pathogenic factors and other substances,can significantly reduce the obstinate itch of skin in uremia patients.
