后腹腔镜肾癌根治术治疗T_2期肾癌的疗效观察

Clinical efficacy of retroperitoneal laparoscopic radical nephrectomy for T_2 stage renal cancer

目的探讨后腹腔镜肾癌根治术治疗T2期肾癌的临床疗效。方法选取2006年9月至2012年9月行后腹腔镜下根治性肾切除术治疗的30例T2期肾癌患者(观察组),与同期32例开放性肾癌根治术患者(对照组)的手术时间、出血情况、术后并发症和患者生存率等进行比较分析。结果观察组患者平均手术时间为(187±28)min,明显高于对照组患者的(159±27)min;观察组患者术中出血量为(280±200)ml,明显低于对照组的(440±240)ml;观察组患者住院天数为(9.0±1.0)d,明显低于对照组的(11.0±2.0)d,差异有统计学意义(P<0.05)。观察组患者发生术后并发症5例,其中肠梗阻2例,切口感染1例,肺炎2例。对照组患者发生术后并发症6例,其中切口感染2例,术中出血2例,肺炎1例,肠梗阻1例。两组患者术后并发症发生情况差异无统计学意义(P>0.05)。结论对T2期肾癌患者实施后腹腔镜肾癌根治术治疗,疗效确切,安全可靠,值得推广。

Objective To explore the clinical efficacy of retroperitoneal laparoscopic radical nephrectomy for T2 stage renal cancer.Methods From September 2006 to September 2012,30 patients of T2 stage renal cell carcinoma（observation group） whom received the retroperitoneal laparoscopic radical nephrectomy were collected,and to select 32 cases of open radical nephrectomy patients as control group,to compare the surgery duration,bleeding,postoperative complications and survival rate of patients between the two groups.Results The observation group using laparoscopic radical nephrectomy,the mean operative time was（187±28） min,and the control group was（159±27） min,the observation group was higher than that of the control group;the blood loss of observation group were（280±200） ml,significantly lower than the control group（440±240） ml,the length of stay of observation group were（9.0±1.0） d,significantly lower than the control group（11.0±2.0） d,the difference was statistically significant（P0.05）.The observation group had 5 patients with complications,including two cases of intestinal obstruction,one case of wound infection,two cases of pneumonia;the control group had 6 cases with complications,including incision infection in 2 patients,blood loss two cases,pneumonia one case and of intestinal obstruction one case,postoperative complications has no significant difference between the two groups（P0.05）.Conclusions For T2 stage renal cell carcinoma patients,retroperitoneal laparoscopic radical nephrectomy has good curative effect,and it is safe,reliable,worthy of promotion.
