妇科肿瘤患者手术后并发下肢深静脉栓塞的原因分析及护理措施

The cause analysis and nursing of deep venous thrombosis of lower limb after gynecologic cancer surgery

目的探讨妇科肿瘤手术后并发下肢深静脉栓塞的原因及护理措施。方法对2008年5月至2012年5月收治的128例妇科肿瘤患者术后并发下肢深静脉栓塞的原因进行分析,实施有效的护理措施。对照组64例,采用传统的预防下肢深静脉血栓的方法;观察组64例,在对照组的基础上,术前3d开始使用间歇充气加压装置、术后6h开始皮下注射速碧林,并针对性实施护理干预措施。结果观察组患者手术后发生下肢深静脉栓塞4例(6.3%),对照组患者术后发生下肢深静脉栓塞16例(25.0%),差异有统计学意义(2=4.27,P<0.05)。结论根据妇科肿瘤术后并发下肢深静脉栓塞形成原因,采取有效的预防措施和早期护理干预,可有效地降低其发生率,缩短住院时间,减少患者痛苦,提高患者满意度。

Objective To investigate the causes of deep venous thrombosis of lower limb after gynecologic tumor surgery and nursing measures.Methods To analyze the causes of deep venous thrombosis of lower limb of 128 gynecologic cancer surgery patients admitted to our hospital from May 2008 to May 2012,and implement the effective nursing measures.64 cases of the control group,receiving the traditional way of prevention of deep venous thrombosis of lower extremity.64 cases of the observation group,on the basis of control group,with 3 days before operation the start of the use of intermittent pneumatic compression device,postoperative 6h subcutaneous injection of FraxiParine,and to implement of nursing intervention measures.Results The observation group patients in the postoperative deep venous thrombosis in 4 cases,the incidence rate was 6.25%;the control group patients in the postoperative deep venous thrombosis in 16 cases,the incidence was 25.0%,the difference was statistically significant（x2=4.27,P0.05）.Conclusions According to the cause of formation of deep venous thrombosis of lower limb after gynecologic cancer surgery,take effective measures of prevention and early intervention,can effectively reduce the incidence rate,shorten the hospitalization time,reduce the suffering of patients,reduce the economic burden of patients,improve patient satisfaction.
