Evaluation of GenoType MTBDRplus for drug-resistance detection of Mycobacterium tuberculosis from smear-positive sputum specimens

摘要 目的:评价耐多种药物结核病快速诊断方法(GenoType MTBDRplus)在涂阳痰标本中检测结核分枝杆菌利福平和异烟肼耐药性的效果。方法:对纳入的涂阳痰标本同步进行L-J分离培养、比例法药物敏感性试验和GenoType MTBDRplus检测,以比例法药物敏感性试验结果为判定标准,分析该方法的灵敏度和特异度以及两种方法的一致性。结果:GenoType MTBDRplus检测利福平、异烟肼敏感度分别为90.6%和82.7%,特异度分别为98.7%和97.3%。结论:在涂阳痰标本中应用GenoType MTBDRplus检测结核分枝杆菌利福平和异烟肼耐药性是一种快速、灵敏、特异度高的诊断方法,可在有条件的地区推广使用。 Objective: To evaluate the effect of GenoType MTBDRplus assay for rifampin (RIF) and isoniazid (INH) susceptibility testing on mycobaeterium tuberculosis from sputum specimens. Methods: Conventional drug susceptibility testing and GenoType MTBDRplus assay were used to detect resistance to RIF and INH of smear - positive sputum specimens. The sensitivity and specificity of GenoType MTBDRplus assay were analyzed. Results: The sensitivities of GenoType MTBDRplus assay for detecting RIF and INH were 90.6% and 82.7% , respectively. Their specificities were 98.7% and 97.3% , respectively. Conclusion: GenoType MTBDRplus assay is a rapid method with high sensitivity and specificity for detecting the RIF and INH resistance in smear - positive sputum specimens and should be promoted appropriately.
