摘要 目的讨论改良Stoppa入路在手术治疗骨盆前环损伤的应用经验。方法回顾性分析2011年4月~2012年4月应用改良Stoppa入路治疗的14例骨盆前环损伤患者的治疗效果。14例按Tile分型:A型1例、B型7例、C型6例。本组应用改良Stoppa入路行手术内固定治疗,术后应用Matta影像学评估标准和Majeed功能评分对骨折复位及术后功能进行评价。结果本组病例在手术切口长度、手术时间、术中出血量、术后并发症等因素较传统手术方法更有优势,而患者术后得到较满意的功能。结论改良Stoppa入路有损伤小、出血少、手术时间、显露充分等优点,是手术治疗骨盆前环损伤的一个理想选择。 Objective To discuss the application experience of modified Stoppa approach in treating anterior pelvic ring injury.Methods From Apr.2011 to Apr.2012,14 cases of anterior pelvic ring injuries were treated by the modified Stoppa approach.All 14 cases were classified by Tile classification: 1 case Tile type A,7 cases Tile type B,6 cases Tile type C1.The modified Stoppa approach was adopted for internal fixation.The radiological results were evaluated by Matta method and the post-operative function was evaluated by Majeed method.Results The indicators including surgical incision length,surgical time,intra-operative blood volume,and post-operative complications were more satisfactory in the modified Stoppa approach group than those of the control group.According to the Matta method and Majeed method,all cases obtained excellent post-operative function.Conclusion The modified Stoppa approach has advantages of mini-invasiveness,less bleeding,less surgical time and sufficient exposure,which is an ideal method for treating anterior pelvic ring fracture.
