微创清除术和小骨窗开颅术对高血压脑出血的临床疗效比较

Clinical comparison of minimally invasive surgery and small bone window craniotomy for hypertensive cerebral hemorrhage

目的对比分析微创清除术和小骨窗开颅术治疗高血压脑出血患者的临床疗效差异。方法回顾性分析2005年2月～2012年1月我院收入治疗的50例高血压脑出血患者,根据其所选择的手术方法分为对照组和微创组,对照组24例患者采取小骨窗开颅术;微创组26例患者采取微创清除术,比较两组患者的治疗有效率及平均手术时间、住院时间等相关指标差异。结果微创组共治疗有效22例,总治疗有效率84.6%,对照组共治疗有效15例,总治疗有效率62.5%,(P<0.05);且微创组平均手术时间、住院时间及再出血发生率均明显优于对照组(P<0.05)。结论微创清除术治疗高血压脑出血具有更为优异的临床疗效,且再出血发生率较低,较为安全可靠,值得临床推广应用。

Objective To compare the clinical differences between the minimally invasive surgery and small bone window craniotomy in the treatment of patients with hypertensive cerebral hemorrhage. Methods 50 cases of patients with hypertensive cerebral hemorrhage were divided into the control group and the minimally invasive group, according to their choice of surgical approach from February 2005 to January 2012 in our hospital. The control group （24 patients） was taken a small bone window craniotomy and the minimally invasive group （26 patients） was taken minimally invasive surgery. The treatment efficiency and average operative time, hospital stay and other related indicators difference were compared between the two groups. Results The total effective rate was 84.6% in minimally invasive group（22/26）, the total efficiency was 62.5% in the control group（15/24）（P 〈 0.05）; and the mean operative time, length of stay and rebleeding rate in minimally invasive group were significantly better than those in control group（P 〈 0.05）. Conclusion Minimally invasive in the treatment of hypertensive cerebral hemorrhage have more excellent clinical efficacy and a lower incidence of rebleeding, a more safe and reliable, worthy of clinical application.
