个体化护理干预对社区老年高血压患者血压管理达标率的影响

Effect of individualized nursing intervention on control rate of blood pressure in community elderly hypertensive patients

目的个体化护理干预对社区老年高血压患者血压管理达标率的影响。方法制定个人体化治疗方法,强化健康教育,干预患者不良生活习惯、体重指数和血脂水平等,规范化管理社区老年高血压患者,观察干预前后高血压患者相关指标达标率的变化。结果个体化护理干预前,血压达标率为74.28%,其中患者血清低密度脂蛋白、总胆固醇、甘油三酯、高密度脂蛋白达标率分别为:30%、28%、55.71%和54.00%,吸烟率、体重指数正常率和服药治疗率分别为68.19%、51.43%和81.14%,个体化护理后患者血压达标率升高至85.14%,与干预前比较差异有统计学意义(P<0.05);患者低密度脂蛋白、总胆固醇、甘油三酯和高密度脂蛋白水平达标率升高至64.00%、56.58%、73.14%和78.57%,吸烟率、体重指数正常率和服药治疗率也分别升高至49.71%、70.57%和95.71%,与干预前比较差异有统计学意义(P<0.05)。结论对社区老年高血压患者给予个体化综合护理干预措施,可显著改善患者生活习惯、血脂水水平,提高血压管理达标率。

Objective To observe the effect of individualized nursing intervention on control rate of blood pressure in community elderly hypertensive patients. Methods Used individualized therapeutic program, enhancing health education, improve the bad life habits, body mass index and blood lipid levels, and standardize the hypertension management in community elderly hypertensive patients program. The changes of blood pressure achieved rate were observed before and after nursing intervention. Results The control rate of blood pressure was 74.28% before individualized nursing intervention, and the serum LDL, CHOL, TG and HDL was 30%, 28%,. 55.71% and 54.00%, respectively. The smoke rates, BMI normal rate and medication treatment rate were 68.19%, 51.43% and 81.14%, respectively. But the control rate of blood pressure rose to 85.14% after individualized nursing intervention, there was a significant statistic difference compare to that before individualized nursing intervention（P 〈 0.05）. And the serum LDL, CHOL, TG and HDL rose to64.00%, 56.58%, 73.14% and 78.57%, respectively. The smoke rates, BMI normal rate and medication treatment rate rose to 49.71%, 70.57% and 95.71%, respectively. Conclusion The individualized comprehensive nursing intervention measures is provide for elderly hypertensive patients in community, and can change the bad life habits and blood lipid levels, and improve BP control rate.
