疏肝和胃颗粒对单纯性酸反流大鼠食管下括约肌超微结构及胃肠激素水平的影响 被引量:6

Effects of Shuganhewei granules on the esophageal mucosal histomorphology,lower esophageal sphincter ultrastructure and gastrointestinal hormone in reflux esophagitis rats

摘要 目的:观察疏肝和胃颗粒对单纯性反流食管炎模型大鼠胃液pH值、胆红素和食管下括约肌(LES)超微结构、NO、NOS、GAS、MTL和VIP及血清GAS的影响。方法:采用"不全幽门缝扎+食管下括约肌切开术"制作单纯性酸反流大鼠模型,实验设正常对照组、假手术组、模型组、奥美拉唑组(8.7mg/kg)、疏肝和胃颗粒小、大剂量组(5.4g/kg、10.8g/kg)。所有动物等体积灌胃给药,2ml/200g,1次/d,连续28d。给药结束次日,检测胃液pH值、胆红素水平;剥取食管下段组织和食管下括约肌,光镜和电镜观察食管粘膜和LES超微结构改变,检测食管下括约肌NO、NOS、GAS、MTL和VIP水平;采血分离血清,ELISA法检测GAS。结果:疏肝和胃颗粒(10.8g/kg)可改善胃食管反流模型大鼠食管粘膜炎性、增生和LES肌纤维分布疏散、肌丝和肌节排列紊乱、肌组织溶解等超微结构改变。同时,疏肝和胃颗粒(10.8g/kg)可降低食管组织粘膜损伤指数和炎症积分、胃液胆红素、食管下括约肌NO和NOS水平,升高食管下括约肌GAS、MTL、VIP水平和血清GAS水平,及胃液pH值,与模型组比较具有显著性差异。结论:疏肝和胃颗粒治疗胃食管反流与其改善胃食管反流大鼠食管粘膜炎症、增生和LES肌纤维的变形、溶解,减少胃酸和胆汁分泌,降低食管下括约肌NO和NOS含量,提高食管下括约肌GAS、MTL、VIP水平和血清GAS水平相关。 To study the influence of Shuganhewei particles on gastric pH , bilirubin, esophageal mueosal histomorphology, NO, NOS, Gastrin, Motilin, Vasoactive intestinal peptide of LES, and serum Gastrin in rat with reflux esophagitis. Methods: Reflux esophagitis model was established by pyloric ligation combined cardiomyntomy. Rat were divided into 6 groups : model group, omeprazele group, ShuGanHeWei granules groups( 10.8 g/kg,5.4g/kg), shamoperated group and control group. Equivoluminal drugs were adiministrated intragastie to rats, 2ml/200g, once a day for 28 days. After treatment, Pathological changes and uhrastructure of LES were evaluated by using HE staining and electron microscope respectively every group, NO, NOS, Gastric pH and bilirubin, Gastrin, Motilin and Vasoactive intestinal peptide of LES and serum Gastrin were determined. Results: Uhrastructural damage including hyperplasia and inflammation of esophageal squamous epithe- hum,muscle fiber, myofilament and sarcomere histolysis of LES was improved by ShuGanHeWei granules. In ShuGanHeWei granules group ( 10.8 g/kg) , the levels of Gastric bilirubin, NO, NOS in LES was decreased, the levels of Gastric pH, Gastrin, Motilin, Vasoactive intes- final peptide in LES and Serum GAS were increased ( P 〈 0.05 or P 〈 0.01 ) than that of model group, also. Conclusion: ShuGanHeWei granules can alleviate esophageal mucosal ultrastructural damage, regulate Gastric pH and bilirubin, NO, NOS, Gastrin, Motilin, Vasoactive intestinal peptide levels in reflux esophagitis rats.
