全胸腔镜及其胸腔镜辅助下小切口治疗大疱性肺气肿的比较分析

Comparative analysis of total thoracoscope and video-assisted mini-thoracotomy for bullous emphysema

目的分析全胸腔镜及其胸腔镜辅助下小切口在大疱性肺气肿的临床治疗对比研究。方法参与此次研究的病患为2006年9月至2012年12月在本院确诊为大疱性肺气肿并接受治疗的26例患者,将这26例患者分为各13例的观察组和对照组,观察组的治疗方法是采用胸腔镜辅助下联合小切口进行治疗,对照组是采用胸腔镜做手术治疗,在手术完成后对这两组患者做否存在有漏气、相应并发症出现的情况做观察记录并比较两者的差异性。在半年以上的回访调查中,对接受治疗的患者其肺部功能做相应的检查,检查结果根据相关的肺部功能指标来评定。结果从术后的临床观察中发现两者患者的手术治疗都获得较好的效果,术后漏气现象的出现次数对照组比观察组多,在并发症和不良反应症状的对比中差异没有统计学意义(P>0.05)。结论在大疱性肺气肿的治疗中,可以采用在胸腔镜的辅助下联合小切口来做其治疗工作,这种临床治疗方法更具疗效,并值得在临床上做推广。

Objective To analyze the comparative study on clinical effect of total thoracoscope and video-assisted mini-thoracotomy treating bullous emphysema. Methods 26 patients with bullous emphysema treated in our hospital in 2012 were selected as the research objects and were divided into two groups, the observation group and the control group, each group had 13 cases. The observation group were treated with thoracoscope and video-assisted mini-thoracotomy. And the control group received surgical treatment by using thoracoscope. After operation, compared the differences betwern two groups by the situation of air leakage and complications. For the reunion surveys for more than half of the year, the lung function of the patients receiving treatment was examined, and the examination results were evaluated according to the relevant indexes of lung function. Results The clinical observation after operation indicated that the surgical treatment of two groups had good effect. The frequency of occurrence in the control group was more than that in the observation group. And the comparison of complications and adverse reactions had no statistical significance, P〉0.05. Conclusion Thoracoscope and video-assisted mini-thoracotomy for bullous emphysema has good effect, which is worthy of promotion and application.
