顺时针旋转结肠圈在先天性巨结肠根治术中的应用

The application of rotating colon ring clockwise in the treatment of Hirschsprung disease

目的探讨顺时针旋转结肠圈在先天性巨结肠根治术中的应用和疗效。方法回顾性分析2009年6月—2012年11月收治的12例扩张段延长至横结肠的先天性巨结肠患儿的临床资料,所有病例均彻底切除病变肠管,通过沿顺时针方向旋转结肠圈的方式重建结肠通路,进行巨结肠根治术(改良Soave术),对治疗效果进行总结和评价。结果 12例巨结肠根治术均手术成功,未出现无法吻合而造瘘或术后结肠回缩现象,排便功能良好,术后无便秘复发,近中期随访效果良好。结论顺时针旋转结肠圈治疗先天性巨结肠手术对肠功能影响小,术后恢复快,效果好,并发症少,值得临床推广应用。

Objective To discuss and summarize the application and efficacy of ＂Rotating colon ring clock-wise＇, a new procedure used in the treatment of Hirschsprung disease. Methods The clinical data of 12 patients with Hirschsprung disease （HD） who were treated with Rotating colon ring clockwise＇ surgery in the First Affiliated Hospital of Xinjiang Medical Unversity from June 2009 to November 2012 were ana-lyzed retrospectively. All of 12 cases with long dilated segment up to the mid-transverse colon underwent pull-through procedure （modified Soavers operation） and the procedure of rotating colon ring clockwise. Results All the patients were cured successfully in the procedures. There were no recent complications and recurrent constipation found during the short and moderate-term follow-ups. Conclusion Rotating co-lon ring clockwise is an effective and convenient procedure for long dilated segmental HD. It is a comple-ment to the trans-anal colonic pull-through procedure, and worthy to be clinically popularized and applied.
