大承气汤保留灌肠对脓毒症患者血清免疫学指标的影响

Effect of Retention-enema with Dachengqi Decoction on Serum Immunology Indexes in Patients with Sepsis

目的观察大承气汤保留灌肠对脓毒症患者血清免疫学指标的影响。方法 30例脓毒症患者随机分为对照组和治疗组,均行西医常规治疗,同时治疗组给予大承气汤保留灌肠,疗程为7d,并进行疗效判定、中医证候积分、APACHEⅡ评分比较,检测血清免疫学指标免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、补体C3、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)的水平。结果与对照组比较,治疗组疗效显著,治疗后中医证候积分和APACHEⅡ评分均明显下降(P<0.05,P<0.01),血清IgA、CRP、TNF-α水平均明显降低(P<0.05)。结论大承气汤保留灌肠能提高脓毒症的疗效,并可显著降低血清免疫学指标水平。

Objective To observe the effect of retention-enema with Dachengqi decoction on immunology inde- xes in patients with sepsis. Methods Thirty patients with sepsis were randomly divided into treatment group and control group. Regular therapy was administered to all the included patients. Additionally, patients in the treatment group received retention-enema with Dachengqi decoction for 7 days. The curative effect, TCM syndrome scoring and APACHE Ⅱ scoring were evaluated as well as serum immunology indexes including immunoglobulin A（IgA）, immunoglobulin G（IgG）,immunoglobulin M（IgM）, complement C3,C-reactive protein（CRP）, tumor necrosis fac- tor alpha（TNF-α）level were measured. Results Compared with the control group, the curative effect, TCM syn- drome scoring and APACHE Ⅱ scoring were significantly decreased in the treatment group（P〈0.05,P〈0.01）. Serum IgA, CRP, TNF-α levels were also significantly reduced （P 〈 0.05）. Conclusion Retention-enema with Dachengqi decoction can improve the outcome of sepsis and can significantly reduce the levels of serum immunology index.
