一次根治术治疗肛周脓肿的疗效观察

Efficacy observation of one-time radical surgery in the perianal abscess treatment

目的探讨一次性根治术治疗肛周脓肿的疗效。方法选取我院2010年3月～2012年10月间进行诊治的216例急性肛门直肠周围脓肿的患者,将其随机分为两组,对照组的100例患者采用单纯切开引流术,观察组的116例患者采用一次性根治切开术,对两组患者的创面愈合时间进行观察,同时比较两组患者术后两年内出现肛瘘及脓肿复发的情况。结果创面愈合时间观察组为(24±4)d,对照组为(20±6)d,比较两组患者的创面愈合时间观察组长于对照组,且差异有统计学意义(P<0.05);两组患者术后两年内出现瘘管或者脓肿复发情况观察组有4例,对照组有70例,比较两组患者出现瘘管或者脓肿复发患者数观察组明显少于对照组,且差异有统计学意义(P<0.01)。结论对于肛门周围脓肿患者采用一次性根治术治疗比单纯切开引流术所需的愈合时间长,但是能够显著降低患者出现瘘管以及复发脓肿的几率。

Objective To discuss the perianal abscess patients by one-time radical surgery in the clinical treatment effect.Methods The patients in our hospital from March 2010 to October 2012 between the diagnosis and treatment of 216 cases of acute perianal abscess,which were randomly divided into two groups,100 patients in the control group using simple incision and drainage,the observation group of 116 patients with primary radical incision,two groups of patients with the wound healing time at the same time two years were observed,compared two groups of patients with postoperative recurrence of anal fistula and abscess occurred within.Results In the observation group,wound healing time was（24±4） days,the control group was（20±6） days,compared two groups of patients with the wound healing time,there was a statistically significant difference（P＆lt;0.05）.Patients in the observation group after two years of 4 case patients with fistula and abscess recurrence phenomenon,the patients of control group had 70 patients with fistula or abscess recurrence after two years,compared two groups of patients with fistula formation and recurrence of perianal abscess,there was a statistically significant difference（P＆lt;0.01）.Conclusion The perianal abscess patients by primary radical operation in the treatment of than simple incision and drainage for long time for healing,but can significantly reduce the probability of recurrence in patients with fistula and abscess.
