胎儿单脐动脉66例临床研究

Analysis of 66 fetal single umbilical artery

目的探讨超声诊断胎儿单脐动脉预测胎儿异常的价值。方法 2002年2月至2010年10月在我院应用超声检测胎儿单脐动脉66例,行胎儿染色体检查21例,分析单脐动脉与染色体异常及胎儿异常的关系。结果产前超声诊断胎儿单脐动脉66例,包括左侧46例(69.7%),右侧20例(30.3%)。单纯性单脐动脉38例(57.6%),合并其他畸形28例(42.4%),其中泌尿系统畸形11例(39.3%),心血管系统畸形6例(21.4%),中枢神经系统畸形4例(14.3%),唇裂畸形3例(10.7%),其他畸形4例(14.3%)。行染色体检查21例,发现染色体异常3例(14.3%),包括18-三体综合征2例,21-三体综合征1例,均合并其他畸形。结论胎儿单脐动脉左侧发生率高于右侧,约42.4%合并其他畸形,大部分单纯性单脐动脉胎儿结局良好。

Objective： To study the value of single umbilical artery（SUA） in predicting fetal malformations.Methods： SUA was diagnosed in 66 cases by ultrasound,from Feb.2002 to Oct.2010.21 cases were accepted karyotype examination,the association about karyotype anomalies,and outcome of fetuses with SUA were analyzed.Results： Through Doppler SUA were diagnosed in 66 cases.The left side in SUA was 46（69.7%）,and the right 20（30.3%）.SUA as an isolated finding occurred in 38（57.6%） cases,and 28（42.4%） fetuses had associated malformations.The abnormalities included 11（39.3%）.urinary system anomalies,6（21.4%） cardiovascular anomalies,4（14.3%） Central nervous system anomalies,3（10.7%） chapped lips,4（14.3%） others anomalies.Chromosomal defects were diagnosed in three of 21 cases karyotype examination,including two cases of trisomy 18,one case of trisomy 21 of other defects.Conclusion： The left side is more frequent than the right in SUA.About 42.4% of the fetuses with SUA are associated with fetal anomalies.The fetal outcome is normal in most of the cases with isolated SUA.
