134例糖调节受损人群的中医体质临床观察研究

Clinical Observation on Constitution of 134 Patients with Impaired Glucose Regulation

目的:观察不同体质人群在糖调节受损中的比例及1年后糖尿病的转化率,探讨糖调节受损患者的体质特点,为中医早期"治未病"的临床干预提供依据。方法:收集资料完整的糖调节受损病例共计134例,根据《中医体质分类与判定》标准将患者进行中医体质分类;1年后,随访受试者,并进行相关临床实验室检查,统计不同体质受试者的DM年转化率。结果:134例IGR患者中,痰湿质患者所占比例最大,随后依次为:阴虚质、湿热质、气虚质、血瘀质、气郁质;在1年之后的复查中,DM年转化率由高到低依次为:湿热质、痰湿质、气郁质、阴虚质、气虚质。结论:在罹患糖调节受损的患者中,痰湿体质、阴虚体质、气虚体质、湿热体质占多数;其中,痰湿体质和湿热体质的糖调节受损患者更倾向于转化为糖尿病,这可能与肥胖引起胰岛素抵抗,使糖代谢异常加速,最终导致糖尿病的发病有关。

Objective： To observe the proportion of the different body constitution in the impaired glucose regulations（IGR）,and the transformation efficiency to the diabetes mellitus（DM）,and investigate the feature of the IGR with constitution in Chinese medicine to supply proof to Chinese medicine prevention in clinic.Methods： Collected 134 cases with completed data,we classified to different constitutions by＂the standard classification of traditional Chinese medicine＂,and calculated the DM transformation efficiency from IGR with different constitution after 1 year.Results： In the 134 IGR patients,the phlegmatic hygrosis constitution is the largest proportion,and the sequential is： Yin asthenia constitution,humit heat constitution,vital energy deficiency constitution,blood stasis constitution and depressed vital energy constitution.The sequence of year DM transformation efficiency in turns is： humit heat constitution,phlegmatic hygrosis constitution,depressed vital energy constitution,yin asthenia constitution and vital energy deficiency constitution.Conclusion： The phlegmatic hygrosis,yin asthenian,humit heat,vital energy deficiency,blood stasis and depressed vital energy constitution are the majority in IGR.The humit heat and the hlegmatic hygrosis constitution are prone to invert to DM.It may be related to the fat reducing insulin resistance,accelerating the abnormality of glycometabolism,and inducing the diabetes mellitus.
