白内障超声乳化联合房角分离术治疗急性闭角型青光眼疗效观察

Clinical observation on curative effect of phacoemulsification combined with goniosynechialysis for treatment of angle-closure glaucoma

目的观察白内障超声乳化术联合房角分离术治疗急性闭角型青光眼的疗效。方法回顾分析2008年9月至2012年6月我科收治的急性闭角型青光眼急性发作期合并膨胀期白内障65例65眼，房角粘连关闭范围为180°～270°，药物控制眼压后行白内障超声乳化联合人工晶体植入术及房角分离术，术后随访6～12个月。结果65眼（100％）术后最佳矫正视力均较术前有不同程度提高，64眼（98．5％1术前房角关闭部位全部开放，术后眼压较术前明显降低，手术前后眼压比较t值为4．26，差异有统计学意义（P〈0．05）。结论白内障超声乳化联合房角分离术治疗急性闭角型青光眼疗效显著，但应注意适应症的选择。

Objective To investigate the clinical effects of phacoemulsification combined with goniosynechialysis for treatment of angle-closure glaucoma. Methods Retrosptive study was carried out to analyze 65 cases （65 eyes） of angle- closure glaucoma complicated with cataract, who were admitted in our department from September 2008 to June 2012. They were examined with gonioscope, and underwent phacmulsification and intraocular lens （IOL） implantation combined with goniosynechialysis after their intraocular pressure got normal. And then we had a follow-up about 6 - 12 months. Results The optimal corrected vision of 65 eyes （100%） improved in different degrees and the anterior chamber angles of 64 eyes （98.5%） were completely open. The intraular tension of 65 eyes remarkably decreased compared with that before surgery. There was significant difference between the two groups before and after surgery （t=4.26, P〈0.05）. Conclusion Phacoemulsification combined with goniosynechialysis has significant efficacy in treating angle-closure glaucoma, while the indication of surgery should be properly determined.
