食管多通道腔内阻抗技术在胃食管反流病诊断中的应用 被引量:3

摘要 目的探讨针对胃食管反流病(GERD)诊断中食管多通道腔内阻抗技术(MII)的医学价值。方法抽取本院2011年1月-2012年1月40例GERD,通过MII与内镜检查、食管测压、24h食管pH监测等诊断方法的比较,来确定MII的诊断价值。结果 MII相对于内镜检查对非糜烂性反流(NERD)有更好的诊断价值,两者对NERD检出率比较具有统计学意义(χ2=20.83,P<0.01),RE检出率比较差别有统计学意义(χ2=6.86,P<0.01);MII相对于食管测压能够更好的检出反流情况,可分析出食管蠕动性收缩与食团输送的关系;食管多通道腔内阻抗技术(MII)联合24h食管pH监测相对于单独24h食管pH监测对pH>4的反流有更好的诊断价值,对pH>4非酸反流的检出率情况比较,差别有统计学意义(χ2=9.60,P<0.01)。结论 MII是一种针对胃食管反流病(GERD)诊断的新型技术,其诊断价值较高,值得进一步临床推广使用。 Objective To study the medical value of esophageal muhichannel intraluminal impedance (MII) in the diagnosis of gastroesophageal reflux disease (GERD). Methods A total of 40 cases of patients with GERD from January 2011 to January 2012 in our hospital were selected for observation, compared the diagnostic methods of esophageal multichannel intraluminal impedance, endoscopic examination, esophageal manometry and 24 h of esophageal pH monitoring to determine the diagnostic value of esophageal multichannel intraluminal impedance. Results Esophageal multichannel intraluminal impedance had better diagnostic value than endoscopic examination in the diagnosis of nonerosive reflux disease (NERD), compared the detection rates of NERD(x/=20.83 ,P 〈 0.01), compared the detection rates of RE (X2=6.86,P 〈 0.01), the differences were statistically significant; esophageal multiehannel intraluminal impedance had better diagnostic value than esophageal manometry in the diagnosis of reflux, and it could analyzed the relations of esophageal peristalsis contractions and food transport; esophageal multichannel intraluminal impedance plus 24 h esophageal pH monitoring had better diagnostic value than single 24 h esophageal pH monitoring in reflux when pH〉4, the difference was statistically significant(x2=9.60 ,P 〈 0.01). Conclusion Esophageal multichannel intraluminal impedance is a kind of new technology in the diagnosis of GERD, its diagnostic value is high and worthy of further clinical promotion.
