晚钠电流与心脏收缩-舒张功能

摘要 慢性心力衰竭在发生、发展过程中伴有显著的心电活动和收缩-舒张功能异常。晚钠电流(INa-L)因其失活缓慢的特点,可增加Na+流入细胞和增强心室复极的频率依赖性,影响平台期多种离子通道和离子交换体的活动过程,引起动作电位后除极。通过增强的反向Na+-Ca2+交换活动,进一步引起细胞内Na+和Ca2+平衡紊乱及心脏舒缩活动异常。多种因素调节晚钠通道的功能重构。使用高选择性阻断INa-L的药物如雷诺嗪可减少后除极所诱发的触发活动,减轻衰竭心脏的收缩和舒张功能障碍,可作为辅助心力衰竭治疗的药物。
