经皮刺激迷走神经左侧耳支拮抗快速心房起搏对肺静脉及心房有效不应期的影响

摘要 目的探讨经皮刺激迷走神经左侧耳支(TVNS)拮抗快速心房起搏对肺静脉及心房有效不应期的影响。方法 14只成年犬,麻醉后双侧第4肋间开胸,心房快速起搏6 h诱导心房急性电重构。犬随机分为:对照组(n=7)和TVNS组(n=7)。TVNS组在心房快速起搏时同时给予20%阈强度的TVNS,TVNS引起窦性心率减慢的最低电压为阈强度电压。在基础状态和每小时末测定心房和肺静脉各部位的有效不应期(ERP)。结果对照组中,心房快速起搏6 h导致各部位ERP逐渐降低,ERP离散度变大。TVNS组中,给予TVNS后抑制了心房快速起搏6 h所导致的ERP、ERP离散度的变化。结论 TVNS可拮抗快速心房起搏对肺静脉及心房有效不应期的影响。 Objective To investigate the inhibitory effects of transeutaneous electrical stimulation in left auricular branch of the vagus nerve on atrial fibrillation induced by six hours atrial pacing. Methods Fourteen adult dogs were underwent intercostal thoracotomy after anesthesia from both sides, and then were randomly divided into two groups. The control group (n = 7 ) was only received 6 hours rapid atrial pacing(RAP) ; The TVNS group (n = 7 ) was applied 6 hours RAP and concomitant 20% threshold TVNS. The threshold was the lowest voltage that slowed down sinus rhythm via TVNS. Effective refractory period (ERP)was measured at mutiple atrial and pulmonary vein sites by each hour. Results In the control group, ERP progressively shortened, while the ERP dispersion progressively increaseds; In the TVNS group, 6 h concomitant RAP+TVNS did not induce any significant change in ERP and ERP dispersion. Conclusions TVNS can suppress atrial fibrillation induced by 6 h RAP.
