反复胚胎种植失败患者杀伤细胞免疫球蛋白样受体基因多态性的研究

摘要 目的:探讨杀伤细胞免疫球蛋白样受体(KIR)基因多态性与反复胚胎种植失败的关系。方法:采用序列特异性引物聚合酶链反应法对研究组46名反复胚胎种植失败患者和对照组16名一次体外受精-胚胎移植即获得妊娠并分娩活婴的患者进行KIR基因多态性检测,并采用SPSS17.0软件进行分析。结果:①研究组的KIR2DS3和KIR3DS1基因频率较对照组高(26.09%,41.30%vs 6.25%,25.00%),而KIR2DS4*003-007的基因频率则低于对照组(36.96%vs 62.50%)。②研究组KIR单倍型B占56.52%,高于对照组的43.75%。③研究组携带2~5个活化性KIR基因比例均高于对照组(依次为17.39%vs12.50%、4.35%vs 0.00%、28.26%vs 25.00%、6.52%vs 6.25%)。上述各观察指标两组间均无统计学差异(P>0.05)。结论:活化性KIR基因频率、活化性KIR基因数量、KIR单倍型B频率在反复胚胎种植失败患者中均有增加趋势,但无统计学差异,有待扩大样本量进一步统计分析。 Objective:To investigate the correlation between the killer cell immunoglobulin-like receptor(KIR) genes polymorphism and the repeated implantation failure.Methods:The study group,constituted of 46 repeated implantation failure patients,was matched to 16 controls who got a pregnant and living birth from the first in vitro fertilization embryo transfer cycle.The genotyping of KIR gene detected by a polymerase chain reaction using sequence specific primers method between the two groups was analyzed by the SPSS17.0 software.Results:①The gene frequency of KIR2DS3 and KIR3DS1 were higher in the study group compared with that of the control group(26.09%,41.30% vs.6.25%,25.00%),while the gene frequency of KIR2DS4*003-007 was lower than that of control group(36.96% v.s.62.50%).②The study group’s KIR genes frequency of B haplotype was 56.52%,which was higher than that of control group’s 43.75%.③Compared with controls,increased percentages of carrying 2 to 5 activating KIR genes was observed in repeated implantation failure patients(the percentage successively were 17.39% vs.12.50%,4.35% vs.0.00%,28.26% vs.25.00%,6.52% vs.6.25%).All of those observation indexes above referred showed no significant difference between two groups(P>0.05).Conclusion:All of the activating KIR genes’ frequency,the activating KIR genes’ number and the frequency of B haplotype show an increased tendency in the repeated implantation failure patients.But there is no significant difference.It’s expected to enlarge the sample size for a further statistic analysis.
