微波消融联合^131I肿瘤细胞核人鼠嵌合单克隆抗体注射液治疗对肺癌患者免疫功能的影响

Influence of the treatment of microwave ablation combined with 131I tumor cells human mouse chimeric monoclonal antibody injection on the immune function of patients with lung cancer

摘要 目的探讨微波消融治疗、^131I肿瘤细胞核人鼠嵌合单克隆抗体注射液^(131I-chTNT)放射免疫治疗及二者联合治疗对肺癌患者免疫功能的影响及其抗肿瘤机制。方法将50例肺癌患者采用数字表法随机分为i组:单纯微波消融治疗组17例、单纯^131I-chTNT放射免疫治疗组15例、微波消融联合^131I—chTNT放射免疫治疗组18例,检测所有患者接受治疗前后T淋巴细胞亚群、NK细胞活性和白细胞介素(IL)-2、IL-10、IL-12表达、干扰素1(IFN-1)、肿瘤坏死因子α(TNF—α)的变化。结果三组患者治疗后的CD4+值、CD4+/CD8+值、NK细胞活性均高于治疗前(P〈0.05),且联合治疗组与两个单纯治疗组比较差异有统计学意义(P〈0.01)。i组患者治疗后的IL-2、IL-12、IFN-γ、TNF-α水平与治疗前比较,升高有统计学意义(P〈0.05),但组间比较差异无统计学意义(P〉0.05)。结论微波消融联合^131I-chTNT放射免疫治疗具有提高免疫功能的作用,能最大程度地提高肺癌综合治疗疗效。 Objective To investigate the mechanism of anti-tumor in lung cancer patients and their effects to immune system undergoing with mimowave ablation treatment, radioimmunotherapy of 131I tumor cells human mouse chimeric monoclonal antibody injection (131I-ehTNT) and combined treatment of two anterior method. Methods The 50 cases of lung cancer were divided into three groups randomly, 17 cases were in the group of simple microwave ablation treatment, 15 cases were in the group of simple radiotherapy immunotherapy, 18 cases were in the group of combined microwave ablation treatment with 131I-chTNT radioimmunoassay. During the study, the T lymphocyte subsets, the activity of NK cells, the expressing of interleukin (1L)-2, IL-10, IL-12, the changing of the interferon γ (IFN-γ) and tumor necrosis factor α (TNF-α) were detected and compared of before and after treatment on all the patients. Results The CD4+ value, CD4+/CD8+ value and the activity of NK cells in these three groups after treatment were higher than that of before treatment (P 〈 0.05), there was statistic difference between the combined treatment group and two simple treatment groups (P 〈 0.01). The expression of IL-2, IL-12, IFN-γ TNF-α in three group patients after treatment were higher than that of before treatment, which had statistical significance (P 〈 0.05), while there were no significant differences between groups (P 〉 0.05). Conclusion The method combined microwave ablation treatment with 131I-chTNT radioimmunoassay has efficiency to improve the immune function, which could improve the comprehensive therapeutic effect of lung cancer excellently.
