儿童多发性大动脉炎58例临床特点分析

Clinical analysis of 58 children with Takayasu's arteritise

目的分析儿童多发性大动脉炎的临床特点及治疗转归，防止误诊漏诊。方法对58例多发性大动脉炎患儿的临床表现、实验室及影像学检查、治疗转归进行回顾性分析。结果58例中男24例，女34例；年龄3～16岁。常见症状为高血压、血管杂音、无脉或脉弱。炎症活动指标为血沉增快、c反应蛋白升高。46例接受激素和（或）免疫抑制剂治疗，有效率55．2％，住院期间无死亡。结论儿童多发性大动脉炎以高血压、血管杂音、无脉或脉弱为主要临床表现，对于高血压的儿童应注意四肢脉搏及血管杂音检查，测量四肢血压，注意大动脉炎的可能，防止误诊漏诊。

Objective To analyze the clinical character and therapeutic effect of Takayasu＇ s arteritis. Method Retro- spective data of 58 children with Takayasu＇ s arteritis were analyzed. Their clinical manifestation,laboratory test, im- aging tests, treatment and prognosis were summarized. Result The age of children varied from 3 years to 16 years and 24 children were male while 34 female. Hypertension, vascular bruits, and acrotism or pulseless were the common symptoms. The active indices of inflammation were increased, such as ESR and CRP. Forty-six children were treated with steroids and/or immunosuppressives,with an effective rate of 55.2%. None children died during hospitaliza- tion. Conclusion Principal clinical manifestation of Takayasu＇ s arteritis are hypertension, vascular bruits and acro- tism or pulseless. Children with hypertension should be given comprehensive examination to rule out Takayasu＇ s ar- teritis, especially examination of pulse and blood pressure of arms and legs and listen to artery.
