三维外固定支架矫治踝关节瘢痕性挛缩畸形

摘要 目的:基于Ilizarov技术的张力—应力法则,运用三维外固定支架矫治踝关节瘢痕性挛缩畸形,探讨此手术方式的临床疗效及特点。方法:收集我科于2009年6月至2012年7月所收治的踝关节瘢痕性挛缩畸形患者23例,依据Ilizarov的张力—应力法则,均采用可调控的三维外固定支架进行矫治。术中将畸形踝关节固定在最大矫正位置,术后3~5d开始行外固定支架调整。矫形时每天牵伸1mm,直到畸形完全矫正并过伸5~10°。矫形过程中患肢可适当行负重锻炼,畸形矫正后外固定支架仍继续固定4周,外固定支架拆除后佩戴支具行走6个月。结果:23例患者中随访至少5个月以上17例,共19个关节,失访6例,随访时间5~30个月,平均(16±8)个月,三维外固定支架牵伸时间3~8周,平均(5±2)周。术后均未出现严重并发症,且较术前相比,所有踝关节功能均有明显改善。结论:基于Ilizarov技术的张力—应力法则,运用可调控的三维外固定支架治疗瘢痕性踝关节挛缩畸形,方法简单、适用性广、效果满意、并发症少,符合畸形矫正的生物学要求,是一种实用且可靠的手术方式。
