髂内动脉或子宫动脉栓塞与子宫动脉结扎治疗产后出血的疗效对比

Comparison of therapeutic effects between internal iliac artery or uterine artery embolization and uterine artery ligation on patients with postpartum hemorrhage

目的:观察并比较髂内动脉或子宫动脉栓塞与子宫动脉结扎治疗产后出血的疗效。方法:将60例妇产科治疗的急诊大出血患者按数字随机分为两组,对照组(30例)采用子宫动脉结扎术,治疗组(30例)采用髂内动脉或子宫动脉栓塞术,观察两组的临床指标及疗效。结果:治疗组的手术时间、止血时间、出血量、出血时间明显低于对照组(P<0.05);两组治疗前的收缩压、舒张压、心率均无明显差异(P<>0.05),治疗后,两组的差异显著(P<0.05);治疗组子宫切除发生率6.7%明显低于对照组的20.0%(P<0.05);两组术后的平均体温无明显差异(P<0.05);随访后,治疗组的术后生活情况优于对照组(P<0.05)。结论:髂内动脉或子宫动脉栓塞治疗产后大出血疗效显著,具有并发症低、止血效果好、操作简单、保留子宫、安全可靠性高等优势,值得在临床上推广。

Objective：To observe and compare therapeutic effects between internal iliac artery or uterine artery embolization and uterine artery ligation on patients with postpartum hemorrhage, nethotls：60 patients with postpartum hemorrhage were randomly divided into control group and treatment group. The patients in control group （30 cases） were given uterine artery ligation; whereas those in the treatment group （30 cases） were treated by internal iliac artery or uterine artery embolization. Then clinical indicators and curative effects of two groups were observed. Results ： Operation time, hemostasis time and blood loss, bleeding time in treatment group were significantly lower than those in control group （P 〈 0.05）. Before the treatment, there were no significant differences in systolic pressure, diastolic pressure and heart rate between the two groups ; however, the differences after the treatment were significant （P 〈 0.05）. Hysterectomy rate （6.7%） in treatment group was obviously lower than that in the control group （20.0%） （P 〈0.05） ; The average temperature of two groups had no obvious difference after the operation （ P 〈 0.05 ）. Postoperative life of treatment group was better than that of control group （P 〈 0.05 ） through follow - ups. Conclusions ： The internal iliac artery or uterine artery embolization in the treatment of the patients with postpartum hemorrhage can reserve uterus, has the advantages of good therapeutic effects, a few complications, good hemostatic effects, simple operation, and high safety and reliability, and is worth being used in clinical fields.
