前列腺增生和前列腺癌组织细胞凋亡及Bcl-2、Bax基因表达的研究

Study of apoptosis and expression of Bcl-2、Bax genes in benign prostatic hyperplasia and prostatic cancer

目的 探讨良性前列腺增生 (BPH)和前列腺癌 (Pca)组织细胞增殖和凋亡及Bcl 2、Bax蛋白表达的临床意义。 方法 用核酸末端原位标记方法检测BPH和Pca标本各 2 3例及 5例正常前列腺 (NP)标本 ,观察细胞凋亡情况。用ABC免疫组化法研究三种组织中PCNA、Bcl 2、Bax表达及特征。 结果 BPH和Pca的增殖细胞指数 (PI)及凋亡细胞指数 (AI)较NP明显增高 (P <0 .0 1) ,但AI/PI却显著下降 (P <0 .0 1) ,其中Pca的PI和AI又较BPH明显增高 (P <0 .0 1) ,而AI/PI却明显下降 (P <0 .0 1)。NP组织Bcl 2表达阳性 ,Bax表达阴性。BPH和Pca组织Bcl 2蛋白阳性率分别为82 .6 %和 5 2 .2 % (P <0 .0 5 ) ,Bax蛋白阳性率分别为 34.8%和 87.0 % (P <0 .0 1)。Bcl 2表达随Pca病理分级增加而明显降低 (P <0 .0 5 )。 结论 细胞增殖增加和凋亡的相对减少参与了BPH和Pca的发生发展过程。Bcl 2和Bax在BPH和Pca的发生发展过程中起重要作用 。

Objective To investigate the expression of Bcl 2、Bax protein in BPH and Pca. Methods A total of 23 cases of BPH,23 cases of Pca and 5 cases of normal prostate(NP) tissue specimens were studied. The apoptosis status was detected by in situ end labeling of fragmented DNA. The expressions of PCNA、Bcl 2 and Bax protein were studied in BPH、Pca and NP tissues by means of avidin biotin complex immunohistochemistry. Results The proliferation index(PI) and the apoptosis index(AI) of BPH and Pca were much higher than that of NP ( P <0.01) and the AI/PI much lower( P <0.01).The PI and AI of Pca were much higher than that of BPH ( P <0.01) and the AI/PI much lower ( P <0.01). Expression of Bcl 2 were positive and expression of Bax were negative in NP tissues. The positive rate of Bcl 2 protein was 82.6% and 52.2% in BPH and Pca ( P <0.05), that of Bax protein being 34.8% and 87.0% ( P <0.01). The Bcl 2 positive rate decreased with the advancement of the pathological grades in Pca. Conclusions The results suggest that both high proliferation rate and relative low apoptosis rate are associated with the pathogenesis of BPH and Pca. The apoptosis genes Bcl 2 and Bax play an important role in the development of BPH and Pca and in the regulation of prostatic apoptosis.
