瑞芬太尼靶控输注辅助腰硬联合麻醉在妇科腔镜手术中的应用 被引量:7

摘要 目的:观察瑞芬太尼靶控输注辅助腰硬联合麻醉在妇科腔镜手术中的应用效果。方法:选择ASA I~II级,拟行子宫肌瘤挖除术的女性患者90例,随机分为3组(n=30):Ⅰ组为单纯腰硬联合麻醉组,Ⅱ组为瑞芬太尼靶控输注辅助腰硬联合麻醉组,Ⅲ组为气管插管静脉复合全麻组。观察麻醉前(T1)、气腹后30 min(T2)、放气腹后30 min(T3)三个时点的血流动力学、血气分析、肩背部牵涉痛以及应激反应指标皮质醇、血糖的变化。结果:T2时点,Ⅱ组跟Ⅰ组比较,肩背部放射痛明显降低(P<0.05);T2时点,Ⅱ组跟Ⅲ组比较,血压下降、pH值降低、PaCO2升高,但变化均在能耐受的生理范围之内;Ⅰ组、Ⅱ组较Ⅲ组的皮质醇、血糖浓度在T2、T3时点均降低。结论:瑞芬太尼靶控输注辅助腰硬联合麻醉较气管插管全身麻醉更能有效抑制应激反应,可以作为妇科腹腔镜手术的麻醉选择,但使用时应注意加强呼吸、循环功能监测管理。
