应用抗菌药物继发真菌感染临床分析

Clinical analysis of secondary fungal infections due to use of antibiotics

目的探讨应用抗菌药物继发真菌感染的情况,以加强抗菌药物的合理化及规范化应用。方法对2008年8月-2011年8月出院患者真菌培养呈阳性病例的临床资料进行回顾性分析。结果单纯性由抗菌药物所引发真菌感染占医院同期发生真菌感染的67.2%,用药天数1～24d,平均用药天数8.1d,在抗菌药物使用过程中,肺部发生继发性真菌感染最多的真菌为白色假丝酵母菌,39例,感染率为43.3%,其次为热带假丝酵母菌感染6例,感染率为6.7%。结论科学、规范控制医院真菌感染的关键环节就是合理应用抗菌类药物,尤其应注意减少>三联抗菌药物的使用。

OBJECTIVE To explore the status of secondary fungal infections induced by the use of antimicrobial agents so as to strengthen the reasonable and standardized use of antibiotics.METHODS The patients with fungal culture positive,who were discharged from Aug 2008 to Aug 2011,were enrolled in the study,then the clinical data of the cases were retrospectively analyzed.RESULTS The cases with fungal infections due to the simple use of antibiotics accounted for 67.2% among the cases with fungal infections over the same period,the medication duration varied between one to 24 days,and the average medication duration was 8.1 days.During the treatment with antibiotics,there were 39（43.3%）cases of patients with the secondary fungal infections caused by the Candida albicans,which were dominant,followed by 6（6.7%）cases of patients with the secondary fungal infections caused by the Candida tropicalis.CONCLUSION The reasonable use of antibiotics is the key point for the scientific and standardized control of nosocomial fungal infections,and great attention should be paid to the reduction of more than the three-drug application.
