睾丸非霍奇金淋巴瘤的MRI表现(2例报告并文献复习)

MRI Manifestations of Testicular non-Hodgkin's Lymphoma (Report of 2 Cases and Literature Review)

目的探讨睾丸非霍奇金淋巴瘤(non-Hodgkin’s lymphoma,NHL)的MR表现。资料与方法回顾性分析2例经手术病理证实的睾丸NHL的MR表现并结合文献复习讨论。结果 2例均为老年男性,年龄分别为72岁和74岁,发现一侧睾丸无痛性肿块入院。MR表现:肿瘤破坏正常睾丸,肿物呈多发结节状和团块状,部分边界欠清,呈等T1、等T2信号,信号均匀,DWI呈均匀高信号,b值升高,肿物仍为高信号,平均ADC值为(0.61±0.12)×10-3mm2/s,增强扫描肿物中等度均匀强化,无液化坏死及囊变区,2例患者肿物均侵犯同侧附睾。结论睾丸非霍奇金淋巴瘤的MR表现具有一定的特征性,结合老年男性患者发现一侧无痛性睾丸肿块,对睾丸NHL的诊断有一定帮助。

Objective To analyze the MR features of testicular non-Hodgkin＇ s lymphoma. Materials and Methods MR features of two patients with testicular non-Hodgkin＇ s lymphoma were reviewed. Results Both of the two patients were elderly men, age of 72 and 74 years old respectively. Initially presented with painless testicular swelling. MR findings： Testicular tumors replaced the testis with one or more focal masses, the major MRI appearances of the tumors were moderate signal intensity on Tl-and T2-weighted images and moderately enhanced after contrast administration.The tumors almost no cystic or necrosis lesions. Marked high signal intensity were showed on DWI whereas the b values were increased. The average ADC value as （0. 61 ±0.12） × 10-3mm2/s .Both of the mass locally aggressived the ipsilateral epididymis. Conclusion Testicular non-Hodgkin＇ s lymphoma has characteristic MR findings,if presented with painless testicular swelling in elder men（60 years and older）, should be considered as testicular NHL.
