超声评价冠心病心外膜脂肪层厚度与颈动脉弹性的相关性 被引量:17

Correlation of Epicardial Fat Thickness and Carotid Artery Elasticity in Patients with Coronary Heart Disease Evaluated by Ultrasound

摘要 目的探讨超声测量冠心病患者心外膜脂肪层厚度(EFT)与颈部血管弹性的相关性。资料与方法 100例患者分为A组(非冠心病组)、B组(单支病变组)、C组(双支病变组)、D(三支病变组),每组各25例,采用超声心动图测量EFT,采用回声跟踪技术测量右侧颈总动脉弹性指标,分析EFT与右侧颈总动脉弹性指标的相关性。结果冠心病组EFT明显高于非冠心病组(P<0.05),且随着冠状动脉病变范围的增加而增加(P<0.05)。EFT与颈动脉内-中膜厚度、僵硬度、弹性模量、脉搏波传导速度呈正相关(r=0.517、0.422、0.542、0.494,P<0.05),与顺应性呈负相关(r=—0.428,P<0.05),与年龄、体重指数、血压、血糖、血脂无明显相关性。结论冠心病患者EFT与颈动脉弹性变化具有良好的相关性,EFT与颈动脉弹性指标均是冠心病的重要预测指标。 Purpose To explore the correlation of epicardial fat thickness (EFT) and carotid artery elasticity in patients with coronary heart disease (CHD) by ultrasound Materials and Methods 100 patients were divided into group A (NCHD group), group B (single-vessel disease), group C (double-vessel disease), and group D (three-vesse disease), with 25 patients in each group. EFT was measured by using echocardiography The right common carotid artery elasticity index was measured by using echo tracking technology. Correlation of EFT and the right common carotid artery elasticity index was analyzed. Results EFT of the CHD group was significantly higher than that of NCHD group (P〈0.05), which increased with degree of coronary artery lesions (P〈0.05). EFT was positively correlated with carotid media thickness, stiffness, modulus of elasticity and pulse wave velocity (r=0.517, 0.422, 0.542, 0.494, P〈0.05), which was negatively correlated with compliance (r= - 0.428, P〈0.05). There was no correlation with age body mass index, blood pressure, blood glucose, and blood lipids. Conclusion There is good correlation between epicardial fat thickness and carotid artery elasticity in patients with coronary heart disease, EFT and carotid artery elasticity index are both important predictors of coronary heart disease.
